  :Under the supervision of the Clinical Program Manager, the Licensed Urgent Response Therapist (URT) provides urgent response to youth and caregivers seeking assistance. This involves phone triage and de-escalation, as well as in-person response and follow up care to ensure increased safety, stability, and linkage to community, health, educational, social, and mental health resources. Additionally, the URT provides clinical assessment and referral.

  • Provide assessment, triage, and crisis response to those seeking support through The Source’s contact methods (phone, text, online, etc.).

  • Coordinate needed response to client/caregiver crisis and involve additional Source roles (UR Coordinators, advocates).

  • Provide short-term follow up services to client/caregiver to further implement techniques from developed safety plans, teach basic coping skills, and facilitate linkage to other providers and community resources.

  • Provide mental health and substance abuse assessment as indicated.

  • Take responsibility for initial safety plan development and implement plan with youth and caregivers.

  • Monitor and ensure effective linkages with community and agency service providers.

  • Must possess a Master’s Degree from an accredited school of Social Work, Counseling or Psychology; and be licensed with the California Board of Behavioral Sciences as an LCSW , LPCC  or LMFT

  • One year of direct practice experience working with seriously disturbed children, adolescents, and families, and experience in engaging parents as partners and working successfully with families from diverse ethnic and cultural groups is preferred.

Unitek College in Sacramento, California is seeking candidates for the position of Program Director for a Physical Therapist Assistant (PTA) Associate Degree Program. The Sacramento campus of Unitek College is currently proceeding through the candidacy stage for programmatic accreditation by the Commission on Accreditation in Physical Therapy Education (CAPTE). Attainment of Candidate for Accreditation status does not assure accreditation by CAPTE. Academic programs at the Sacramento campus are accredited by the Accrediting Commission on Career Schools and Colleges (ACCSC) and the Bureau of Private Post-Secondary Education (BPPE).


 


The PTA Program Director is responsible for (a) adhering to the CAPTE standards on new program development, (b) developing accreditation submission materials/reports for CAPTE and other accrediting agencies, and (c) working alongside the College academic corporate team to meet critical program development milestones. Once the campus gains CAPTE approval, the Program Director will also be responsible for teaching in the PTA Associate Degree Program and oversee program assessment, maintenance of curriculum development and updates, new program initiatives, and assist in the recruitment and retention of PTA students.


 


Minimum Qualifications


Applicants must meet the following criteria for consideration:


·         a minimum of a master’s degree;


·         hold a current license to practice as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) in the state of California or eligible for California licensure;

LINES OF AUTHORITY:


This position is supervised by the Adult Services Clinical Director and/or designated clinical site supervisor or program manager. Unless specifically designated, this position does not supervise any other personnel.


POSITION SUMMARY:


Provides outpatient counseling, and other supportive covered services to eligible adult clients. Maintains client records and required paperwork. An acceptable level of productivity is expected to be maintained as measured by billable hours/service value.


 


PRINCIPAL DUTIES/RESPONSIBILITIES:



  1. Provides brief, solution-focused individual, group and family therapy to a diverse caseload of adult clients, as evidenced by direct observation, treatment team meetings, and clinical record review.

  2. Maintains client records in compliance with Mohave Mental Health Clinic, Inc., State, HCIC and Joint Commission standards, as evidenced by clinical record reviews, peer review, and annual quality profile data. .

  3. Networks with community agencies to provide services to eligible clients, as evidenced by clinical record reviews, direct observation, and treatment team meetings

  4. Utilizes best practice guidelines to ensure appropriate length of time in care, as evidenced by review of assigned caseload, clinical record review and treatment team meetings.

  5. Assists in crisis/intake coverage and completion of pre-petition screenings/social worker evaluations when needed, as evidenced by direct observation.

  6. Provides after-hours and holiday crisis coverage on rotating schedule, as evidenced by review of on-call schedule and corresponding beeper logs.

  7. The employee must work well under pressure meeting multiple and sometimes conflicting deadlines. The employee shall, at all times, demonstrate cooperative behavior with colleagues and supervisors. This is evidenced through direct observation and interviews with co-workers and supervisors.

Provides independent physical therapy management of patients under the direction of the Rehabilitation Services Director or designee. Provides physical therapy services in specialized patient care clinics, and assists in the development, implementation and review of departmental patient care programs.

Initiates discharge planning for physical therapy.   • Provides advice and education to patient and family. Discharges patient from physical therapy when the patient has received maximum benefit from physical therapy. Provides accurate, timely documentation of physical therapy patient management.

Assists in the development, implementation, and review of departmental patient care programs. Performs other duties as needed in the areas of staff education, departmental operations, and departmental projects.   • Acts as an instructor in patient education classes/groups within and/or outside the department based on operational need as defined by departmental leadership. Provides a minimum of one departmental in-service per year as requested by the Rehab Director e.g. journal club presentation, regular in-service presentation. Serves as a clinical instructor to PT students as requested by the Rehab Director. Able to manage complex patients independently with minimal consultation.

The Physical Therapist evaluates and conducts medically prescribed physical therapy treatment programs in an acute care setting . Supervises physical therapy assistants and aides in performing treatment. Supervises students.

Principal Accountabilities

  • Assures efficient management of work processes, by completing tasks and assignments by time standards.

  • Completes post-intervention activities: session closure, documentation, communication, charge capture.

  • Prepares for therapeutic intervention.

Qualifications

Education, including Licensure, Certification, Registration:

Experience:
  • One year of experience in an acute-care hospital, home health or rehabilitation-center setting preferred.

  • Entry-level doctorate (DPT) or transitional DPT, or be within one year of graduation


  • A current license to practice in a state or territory of the United States
