The Program Leader must have a high school degree, general education  diploma, or certificate of completion.  Experience working with adults  with developmental disabilities in employment, education, and recreation  programs is preferred, but not necessary.  The Program Leader must:

Possess a valid California Drivers License and have a good driving record.

Pass Department of Justice and FBI criminal background fingerprint clearance.  Pass a pre-employment physical and drug screening.

Job Description


Our team is gathering qualified candidates for a position at Adventist Health - St Helena. 

Master s Degree in social work Required - LCSW Required 2 years experience in acute or long term medical setting. Provides psychosocial assessments, diagnosis, and treatment, as well as discharge planning to and consultation about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. Functions in a supervisory role as a team leader.Collaborates and facilitates patient discharge plans for those requiring post Hospice or post inpatient care, ensuring that the required paperwork is completed.Assists patient/family/caregiver in determining appropriate level of Hospice care.Initiates a plan of care and appropriate patient teaching. Reassesses plan of care according to the physical and emotional needs of patient.Develops social work Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics. Documents relevant data accurately and in compliance with laws and regulations.Actively participates in training and/or cross-training of duties.Performs other job-related duties as assigned.

 Creekside Behavioral Health a 57 bed inpatient psychiatric facility located in Santa Rosa.

We are seeking Masters level social workers to work with individuals with severe and persistent mental illness in a locked setting. Competence in group and individual therapy, assessment, care planning and engagement are necessary.

Willing to provide supervision toward licensure.
