Position Overview

The Sr. Instructional Designer, Patient Experience reporting to the Sr. Director, Education and Training will be responsible for designing the patient learning experiences influenced by patients, clinicians and Tablo experts. This role is not just a content development role, it is committed to creating elegantly simple experiences for patients to overcome both learning curve and emotional obstacles as they attempt to manage their own care.

Partnering closely with internal and external experts, the Instructional Designer will better understand the needs of patients and develop learning programs that support them through all phases of the learning curve. The learning programs may include web-based learning, instructor-led training, interactive simulations, virtual training, video training, augmented and virtual reality training and assessments.

Job Functions


  • Work with patients and experts to identify emotional and skill-based learning objectives

  • Develop educational models to ensure all content developed is aligned to skills, environment and emotional obstacles required to successfully manage care

  • Independently project manage all phases of learning projects using the ADDIE model: analysis, design, development, implementation & evaluation

  • Conduct needs and gap analysis

  • Simplify complex concepts through the development of video, illustrations and graphics

  • Develop storyboards

  • Develop video or animation scripts

  • Develop eLearning, instructor led, virtual and video training curriculum

  • Evaluate the efficacy of courses, content and program

  • Analyze assessment and evaluation results to recommend design/development improvements

  • Develop cross functional processes and workflows

  • Research and recommend 3rd party training solutions Requirements / Skills

  • Instructional Design/Technologies Master degree preferred

  • Minimum of 5-7 years of relevant professional experience required

  • Must have strong interpersonal and written/verbal communication skills

  • A self-starter who has the ability to prioritize projects and tasks, work with a variety of personalities, and is comfortable with multi-tasking

  • Experience with Design Thinking preferred

Responsibilities

- Be a part of an evolving continuum of care through the use of a holistic approach to the health and wellness of a geriatric population.
- Serve as a clinical leader and collaborate with other leaders to meet the comprehensive health care needs of geriatric residents in a community setting.
- Apply principles of evidence-based practice to promote quality care within the framework of an exceptional, best-in-class care delivery model.
- Provide clinical care through the direct application of the nursing process; perform and document resident assessments, complete Individualized Service Plans, provide or delegate hands-on clinical care and evaluate resident outcomes.

- Coordinate with physicians, select preferred providers and Sunrise team members to champion an integrated approach to resident care and promote positive clinical outcomes

- Hire, train, educate, manage, direct, and monitor the nursing team, medication-care managers and other caregivers.

- Lead the community medication management program through the application of principles of safety in the medication use process.

- Lead clinical quality and process-improvement efforts through the tracking, trending and evaluating of clinical data to recognize clinical excellence and identify areas of opportunity.

Duties include helping to develop and to ensure compliance with policies, customer interaction, direct care of dogs, support for staff in playgroup, managing and mentoring of less experienced staff members. At Sunrise, our Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.

- Build meaningful relationships with a specified number of seniors and their families as you provide assistance with activities of daily living, attend to individual care needs and get to know their unique preferences and personalities - Notify management of changes in condition and recommend adjustments in the level of care and service - Assist residents in life skills and other life enriching activities as indicated on their individual profile - Blends a variety of multi-sensory experiences into the resident's day


- Participates in the development of the Individualized Service Plans (ISP) and monthly updates


- Responsible for their designated group of residents during the shift, knows where their residents are and physically checks on them throughout the shift.

- Responds to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards

Qualifications:
- Dedication to and passion to serve seniors with excellent customer service skills

- High School diploma/GED accepted and may be required per state regulations
- In states where appropriate, must maintain certifications
- Must be at least 18 years of age
- Previous experience working with seniors preferred
- Ability to make choices, decisions and act in the resident’s best interest
- Possess written and verbal skills for effective communication and a level of understanding
- Competent in organizational and time management skills
- Demonstrate good judgment, problem solving and decision making skills
