The rehabilitation specialist supports and motivates program participants in our residential brain injury rehabilitation center as they address cognitive, physical, medical, behavioral and psycho-social rehabilitation programming goals (our employees do not provide direct personal cares - bathing, toileting, etc.). They support program participants as they work to develop strategies to enable them to return to lives of personal meaning where they live, learn, work and recreate in their chosen communities.


We are seeking professionally-minded individuals who are looking for an opportunity to learn and grow with our expanding company. Extensive training is provided and includes national certification in brain injury services. Ideal candidates will have experience in the human services, brain injury, or mental health field or currently seeking a degree in these fields. Positions are available at our Des Moines and West Des Moines location for evening full-time and part-time.


Requirements:



  • Bachelor's degree in Human Services, Psychology, Sociology, or related field preferred (Currently enrolled students are encouraged to apply)

  • Experience working with individuals with brain injury, mental illness or other disability preferred

  • Driving requirement - Must be at least 21 years old with a valid driver's license and have a good driving record in accordance with CNR's policies and procedures

  • Able to pass state and federal background checks, and a pre-employment physical and drug screen are required

  • High school diploma or equivalent required

PROGRAM & POSITION SUMMARY

The Rehabilitation Specialist provides services intended to improve functional and/or daily living skills across a broad range of core life functioning that have been identified to address a behavioral health need in the client’s plan. Rehabilitation services provide assistance in restoring, improving, and/or preserving a client’s functional, social, communication, or daily living skills to enhance self-sufficiency or self-regulation in multiple life domains relevant to the developmental age and needs of the client. While services tend to be provided one to one on campus, they can also potentially be provided in the schools, family homes, and in other community settings.

The Rehabilitation Specialist is a member of the clinical department and reports to the Rehabilitation Supervisor. They work with members of the extended St. Vincent’s treatment team that includes psychiatrists, therapists, TBS clinicians, Equine therapists, and residential staff.

Rehabilitation Specialists work Monday through Friday with a possibility of including a weekend day instead of a weekday. In general, their hours are variable, but will be a variation of 11:00 am - 7:30 pm. 

ESSENTIAL DUTIES & RESPONSIBILITIES

REHABILITATION SERVICES
 

  • Provides mental health services to individuals and groups in the rehabilitation program as assigned.  Substitutes for other service providers when they are absent.

  • Provides documentation for each client following each service within the required time frame and submit billing equal to 50% of the specialist’s work week.

  • Participates in clinical rounds for the purpose of providing observations and recommendations regarding the treatment needs of clients.

  • Collaborates with other care providers on the development of a plan of care and behavior goals. Contributes to and follows plan for partnering with families in the milieu.

  • Participates in Child, Family Team Meetings and other evaluation meetings for the purpose of providing observations and recommendations regarding the treatment needs of clients.

  • Participates in continuous quality improvement activities as assigned.

  • Offers consultation to unit staff, teachers and other significant care providers involved with clients.

  • Provide in-service education as requested.

  • Participate in agency wide program design and evaluation activities as schedule permits.

DEPARTMENTAL ACTIVITIES AND SUPERVISION
 

  • Receives one hour of individual supervision per week.

  • Participates in weekly 2-hour group supervision.

Education & Experience
 

  • BA preferred; two years of relevant experience working with children, youth and families.

  • Experience working with children and youth in Residential or Foster care a plus.

  • Ability to write & communicate clearly.

  • Ability to provide accurate, timely documentation

  • Contractual Rehab Specialist will carry a caseload of 15-30 (depending on your ability to manage caseload).

  • Contractual Rehab Specialist will complete Face-to Face forms for all consumers within the first 5 days of consumers being admitted into the PRP program (must be typed).

  • Contractual Rehab Specialist will complete discharge summaries for consumers within 15 days in the Electronic Health Record.

  • Will provide a DLA’20assessment (adults only).

  • Contractual Rehab Specialist will complete IRP’s within 10 days of a consumer being admitted into the PRP program. Review of plans will be done according to the Beacon schedule and filed in 10 days.

  • Meet with Rehab Coordinators for monthly supervision and provide typed documentation of that meeting.

  • Utilize the agency’s electronic health record (EHR) to document all client services and progress.

We are currently seeking a* Psychiatric Rehabilitation Specialist * to implement the rehabilitation activities outlined in the Clients IRP. This position provides clinical supervision to our direct care staff and provides direct services to clients by providing services to include intake assessments, individual / family / rehabilitation group treatment and crisis intervention.


Primary responsibilities include:



  • Conduct PRP Intake Assessments


  • Provide and coordinate transportation for client activities


  • Complete all documentation related to the provision of services including but not limited to: Contact notes and Individual Rehabilitation Plans (IRP) as needed in our EMR.


  • Submit to OptumHealth for authorization of services.


  • Conduct or assist with on-site rehabilitation activities for clients.


  • Carry a small caseload




  • Provide offsite rehabilitation activities for clients in the community Ensures that all rehabilitation services and activities are focused on the needs and goals identified in the IRP.


  • Ensures that all consents, assessments and plans are completed within the appropriate timeframe identified in COMAR 10.21.29


  • Ensure that direct care staff is in serviced and educated on the IRP or ITRP and guide the direct care staff in activities focused on implementing the Plan.


  • Provide ongoing training and support to direct care staff as needed.


  • Ensures that the encounter requirements are met per regulation for services


  • Ensures that the required documentation is completed by the direct care staff and rehabilitation specialist and contains the required information per regulation (ie.Contact notes, progress notes)


  • Coordinates and Communicates with the PRP Program Coordinator regarding IRP or ITRP and implementation of each client


  • Facilitates and assists in emergency response services as needed according to program policy


  • Conduct daily check in’s with staff.


  • Coordinate with other mental health services client may be receiving


  • All other duties as assigned.

*Use of own vehicle for visits to clients’ homes
*Valid driver’s license

Basic Qualifications


Is a licensed mental health professional as an LGPC, LCPC, LMSW, LCSW-C; OR


ii. Is a rehabilitation counselor who is currently certified by the Commission on Rehabilitation Counselor Certification; OR


iii. Bachelor’s prepared certified psychiatric rehabilitation practitioner currently certified by the U.S. Psychiatric Rehabilitation Association

IFCCS is an integrated, intensive, community-based program, grounded in a philosophy of unconditional commitment to partner with clients and families to strengthen and empower relationships by fostering hope, wellness, and resiliency.   The most important outcome of the IFCCS program is to promote the safety, permanence and well-being of children and Transitional Aged Youth (TAY) involved with the Child Welfare System.   A multi-disciplinary treatment team provides an array of services firmly grounded in the Shared Core Practice Model.  Services are intended to expedite access to ICC and IHBS.


 


Under the direct supervision and leadership of the Program Supervisor the IFCCS Rehab Specialist is a direct service provider who, along with the IFCCS Care Coordinator and Therapist, is a member of the IFCCS team. Participates as member of the Child and Family Team and takes part in developing and implementing the Mental Health Client Treatment Plan (CTP) and the IFCCS Matrix. The IFCCS Rehabilitation Specialist provides direct services and supports to children and their families in a variety of settings. The IFCCS Rehabilitation Specialist is expected to deliver intensive home-based services that are individualized, and strength-based mental health interventions that are designed to ameliorate the mental health condition(s) that interfere with the child’s functioning.


 


DUTIES AND RESPONSIBILITIES:



  • Under the direct supervision of the Program Supervisor and along with the IFCCS Care Coordinator and therapist, takes part in the Engagement process and conducting the strengths-based discovery and assessment with the child and family. Helps them to focus on clarifying those underlying mental health needs that are barriers to the child’s successful functioning.

  • Participates in Child and Family Team meetings.

  • Forms relationships with the child and family and assists them in meeting their goals.

  • Assists the child and family in the home, school and community by providing a variety of supportive Intensive Care Coordination, including urgent service delivery and care planning.

  • Provides Intensive Home Based Services (IHBS) that build on individual strengths, concerns, and needs and that are designed to help reduce the mental health concerns/condition that impair the child’s function, with a balanced focus on the family as a unit.  Some of these services may include skills-building, mentoring, coaching, behavioral support, and psycho-education.

  • Provides Individual, Family and Collateral therapies as required.

  • Maintains a close and supportive relationship with the child and family, program staff, community supports and any others who may be involved with the child and family.

  • Maintains 24-hour on-call availability unless otherwise specified.

  • Responds and intervenes as necessary and appropriate.

  • Completes documentation on all services provided within 24 hours of service provision.

  • Provides Program Supervisor with weekly program status on meeting documentation requirements and timelines.

  • Works collaboratively with staff, family members, public and private agencies and community support systems.

  • Participates in all required trainings.

  • Attends and participates in weekly group and individual clinical supervision.

  • Performs other duties as assigned.

  • Complies with all Federal, state and local laws, rules, and regulations.

QUALIFICATIONS



  • Registered AMFT, ASW or LMFT, LCSW in California and have experience working with the target population of children and families OR

  • Bachelor's degree in relevant field and one to two years experience providing direct services to vulnerable children and families.

  • Must possess sensitivity to family culture, values and beliefs, and be able to operate from a family-centered, strengths-based perspective.

  • Clinical knowledge and expertise within scope of practice in group, family and individual therapy.

  • Community resources in Antelope Valley and surrounding areas.

  • Should have experience working as a member of a team.

  • Experience with DMH paperwork preferred.

  • Must possess good written and verbal communication skills, be organized and be willing to work a flexible and varied schedule, including being available on-call 24/7.

  • Must possess a valid CA driver’s license, be able to drive, and remain insurable.

  • Bilingual helpful

  • Must be willing to transport client/families as approved by supervisor, and as needed and indicated in the IFCCS Matrix.

We are currently recruiting for several Mental Health Rehabilitation Specialist positions for our Short Term Residential Therapeutic Program (STRTP) for adolescents referred through Juvenile Probation and Social Services. The Mental Health Rehabilitation Specialists will work in collaboration with the clinical treatment team to ensure proper delivery of services to the youth in the STRTP Program. If the youth requires services outside the expertise of the agency the Mental Health Rehabilitation Specialist in collaboration with the Mental Health clinician will locate community services to ensure they meet the unique needs of the youth. They will manage the case in coordination with the clinical staff to ensure services to the youth are provided in accordance with the approved case plan, complete intake summaries, provide regular progress notes and will complete transition plans for the youth. They will attend appropriate sessions and represent the youth in CFT meetings to discuss their progress towards reaching their treatment goals in coordination with all stakeholders individuals involved in the case.


Min Education/Experience Requirements: Bachelor’s Degree from an accredited college or university in Child Development, Psychology, and Social Work or other behavioral science field + 4 years experience in a mental health setting OR Current enrollment in a Master’s Degree Program in Social Work, Rehabilitation Counseling, Marriage and Family counseling or psychology each year of enrollment will substitute for a required year of experience and 2-3 years of experience in a mental health setting.

The rehabilitation specialist supports and motivates program participants in our residential brain injury rehabilitation center as they address cognitive, physical, medical, behavioral and psycho-social rehabilitation programming goals (our employees do not provide direct personal cares - bathing, toileting, etc.). They support program participants as they work to develop strategies to enable them to return to lives of personal meaning where they live, learn, work and recreate in their chosen communities.
