Assesses and treats patients referred to the Physical Therapy department. Documents patient evaluation, plan of care, progress and discharge status.

3. Conducts evaluations consistent with and appropriate for patient diagnosis. Designs a treatment plan that is goal oriented based upon interpretation of evaluation results and patient’s personal goals. Establishes goals that are functional, measurable and attainable. Performs re-evaluations per department criteria with modifications/updating of goals as dictated by patient’s progress
4. Assists patient towards goal achievement through effective treatment plan and interventions. Administers individual treatment procedures. Develops home exercises and self treatment plans for the patient
5. Provides education to patient and family, including diagnosis and prognosis of therapy. Recognizes adverse responses or contradictions to therapy and adjust treatment accordingly.
6. Makes timely and appropriate discharge plans and recommendations for the anticipated needs of the patient.
7. Responsible for the accurate and thorough completion of, evaluation reports, progress notes/flow sheets discharge summary and any additional required documentation (e.g. consent, staffing reports, rounds, attendance agreements, patient and family education needs assessment form) in a specified time frame. Documents the occurrence of education by recording occurrence(s) of education and /or home exercise programs provided and reports patient/family return demonstration and /or comprehension of training provided. Submits daily charges that accurately reflect documentation within a specified time frame and assures that documentation is complete prior to discharging patient.
8. Participates in staff orientation, patient staffing and team meetings. Makes appropriate recommendations and referrals for additional services. Education: Requires professional education culminating in a minimum of a bachelor’s degree in Physical Therapy from an accredited academic program.

Experience: Prefer previous experience in a physical therapy clinical setting but will accept a new graduate from an accredited college.
