Our unique Lead Practitioner position grants you with the opportunity to be an integrative ambassador in our store. This role allows you to connect with your community and to build up your own private practice. Our LP’s strive to share and provide training to coworkers, thereby moving us towards the "One Medicine" model and increasing/driving sales. You'll be on the retail floor, engaging with customers, sharing your knowledge and allocating health and wellness solutions. Sales skills are important to succed in this role and having the ability to connect with a wide variety of audiences is essential.

As we're all one team, you’ll learn how to merchandise, stock, order and handle other retail responsibilities in addition to being on the sales floor. You'll also attend a weekly call to engage with our Lead Practitioners in other stores. 


  1. Requirements:


  • Retail and sales professional with experience in exceeding sales goals

  • Completed certification in area of expertise (i.e. herbal medicine, homeopathy, naturopathic medicine, nutrition, traditional Chinese medicine, etc.) or equivalent hands-on experience. Submission of certificate or other credentials will be required

  • Solid understanding of nutritional supplement and botanical medicine uses, dosage and Confidence in speaking with customers and assisting them with their needs while staying within DSHEA guidelines

  • Ability to operate a point of sale system

  • Flexibility in scheduling, including availability for some evenings and weekends

  • Work collaboratively with a multi-disciplinary team to promote the optimal use of antimicrobial agents, reduce the transmission of infection, and educate health professionals, patients, and the public.

  • Provide direct patient care, including treatment assessment and medication management activities.

  • Clinically evaluate all patients in specified area on a daily basis with relevant documentation placed in the medical record regarding pharmacy recommendations, plans, interventions, and medication reconciliation.

  • Pharmacists help to ensure that the prophylactic, empirical, and therapeutic uses of antimicrobial agents result in optimal patient outcomes.

  • Pharmacists provide recommendations to the Pharmacy & Therapeutic Committee to ensure that the number and types of antimicrobial agents available are appropriate for the patient populations served; analyzes data to ensure patient outcome data is used to assess the effectiveness of antimicrobial-use policies throughout the organization;

  • works with the laboratory, infectious disease specialists, and infection preventionists in compiling susceptibility reports (at least annually) for distribution to prescribers to guide empirical therapy;

  • utilizes information technology to enhance antimicrobial stewardship through surveillance,

  • utilization and outcome reporting,

  • the development of clinical decision-support tools; and reduce potential errors and adverse drug events.

  • Pharmacists detect and prevent antibiotic-related drug-drug interactions; monitor time-sensitive automatic stop orders; perform automatic changes from intravenous to oral antibiotic therapy; dose adjust antibiotics in cases of organ dysfunction; optimize therapy for highly drug-resistant bacteria; achieve central nervous system penetration where needed; intervene where therapy might be unnecessarily duplicative; review microbiology cultures to target susceptible therapy of the infecting organism or stop antibiotics when culture results suggest infection unlikely.

  • The primary mission of the Infectious Disease Clinical Pharmacist is to improve patient safety.

  • Provide cost-effective management of drug utilization within the clinic.

  • Monitor process and outcome of patient care relating to drug therapy.

  • Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.

INFECTIOUS DISEASE DEPARTMENT PHARMACIST: A minimum of one-year pharmacy experience in Infectious Disease within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency is required. ASHP accredited PGY-2 Specialty Residency in Infectious Disease or Infectious Disease Pharmacy Fellowship or equivalent experience required. Board certification is preferred. Chief, Department of Pharmacy has option to waive requirements

POSITION SUMMARY


The focus of this position is three-fold:  provide timely, appropriate, and cost-effective drug therapy for health plan members by reviewing prior authorization and medical necessity requests; works to meet CMS regulatory requirements for Medicare Part D; and supports and develops clinical initiatives to achieve HEDIS goals.


 


COMPLIANCE WITH REGULATIONS


 Works closely with pharmacy staff and PBM to ensure that the processes, programs and services are accomplished in a timely and efficient manner in accordance with CHG policies and procedures and in compliance with applicable state and federal regulations including CMS and/or Medicare Part D and Medi-Cal as well accreditation standards such as NCQA.


 


RESPONSIBILITIES



  • Review prior authorization requests and make determinations for drug products according to member benefits and coverage criteria.  


  • Provide clinical guidance and support to the pharmacy benefit specialists.


  • Work collaboratively with interdisciplinary care team in care coordination rounds to optimize patient health outcomes.


  • Research drug information to maintain drug knowledge and disease state understanding for application to prior authorization reviews


  • With the team, develops and implements clinical criteria for the review of non-formulary or non-preferred medications and clinical edits to adjudicate claims and enhance drug utilization


  • Assists in analyzing and researching pharmacy cost trends and clinical program opportunities in managing high-cost therapies.


  • Works to meet CMS regulatory requirements for Medicare Part D, including but not limited to compliance related to formulary administration, the Part D coverage determinations process, Medicare reporting, , Medicare medication safety and overutilization monitoring, and fraud, waste and abuse activities.


  • Takes a lead role in the implementation of new drug-related initiatives and the establishment of new clinical pharmacy services or initiatives as requested.


  • Contributes to the team effort, maintains customer confidence, protects operations, and identifies potential quality issues by attending department meetings; giving and receiving feedback.



  • Performs other duties and assignments as requested.


     


     


    EXPERIENCE/SKILLS





  • Must have a current and unrestricted California pharmacy license.


  • PharmD degree preferred


  • Minimum 1 year in managed care pharmacy; 3 years in retail or PBM experience preferred


  • Bilingual in Spanish, preferred.


  • Experience with Medi-Cal and Medicare preferred


  • Full working knowledge of Microsoft program


  • Knowledge of managed care principles, CPT, ICD-9, HCPCS coding, experience with medical review criteria preferred


  • Ability to communicate effectively verbally and in writing


  • Exceptional telephone and customer service skills


  • Ability to establish effective working relationships with physicians and medical professionals



  • Ability to organize work effectively, determine priorities, and work well independently

Curant Health, Clinical Services department is searching for an PRN MCM Pharmacist to join its team in Smyrna, GA.  This position is primarily responsible for performing medication-related clinical services including but not limited to delivering enhanced Medication Care Management® (MCM) services to patients and prescribers via outbound and inbound communications that optimize therapeutic outcomes for patients. 


Responsibilities:


  • Provide Medication Care Management to pharmacy patients via telephone or other telehealth technologies (such as video conference).

  • Perform comprehensive medication reviews, as part of MCM, to collect patient-specific information and assess medication therapies to identify medication-related issues.

  • Consult with prescribers/healthcare providers to resolve medication-related issues.

  • Create and communicate a Personal Medication Record (PMR) for the patient.

  • Create and communicate a Medication Action Plan for the patient.

  • Maintain proper documentation of MCM activities in software application (i.e. MedPlan).

  • Provide education and training on appropriate use of medications and monitoring devices and importance of medication adherence, while identifying barriers to adherence, and understanding treatment goals.

  • Provide proper follow-up and monitoring with patients and prescribers/healthcare providers.

  • Responsible for interpretation of physician medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies and, based on an assessment of the patient's specific medical needs, making real time interventions with the medical provider and staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy services.

  • Responsible for maintaining accurate and proper patient medication profiles, monitoring to insure the proper choice of drug, dosage form, strength, length of therapy and interaction potential of the prescribed drug.

  • Requires a bachelor's degree and/or an advanced degree in pharmacy with at least 3 years of experience and is licensed to practice in state.

The Staff Pharmacist is responsible for the preparation of nuclear pharmaceuticals in accordance with prescriptions and customer orders and ensures compliance with corporate, state and federal regulations for the safe handling and security of radioactive and biohazardous materials, and provides supervision for pharmacy team.


RESPONSIBILITIES:



  • Draw doses at low energy or PET facilities either manually or with the use of manipulator arms at PET facilities;

  • Compound drugs in compliance with customer order; Elute generators to make material and combine the materials to create correct drug and dose;

  • Perform constancy check on the dose calibrator and the SCA; Ensure equipment, which may include survey meters, single-channel analyzers, and dose calibrators, remains in proper working order; and check for proper functioning and calibration;

  • Conduct quality control on kits and instruments; perform bio-assays, lab wipes & surveys, operate single channel analyzer to determine contamination level, document results; Perform a wide variety of quality assurance tests including: tests for sterility, pyrogenicity, and kryptofix as well gas chromatography tests, thin layer chromatography tests, alumina tests, molybdenum tests and other tests pertaining to both PET and low energy products;

  • Pack and wrap containers and prepare for shipment; Operate survey meters and single-channel analyzer to determine contamination level, document results;

  • Complete bill of lading, placards and DOT documentation pertaining to the packaging and shipping of doses;

  • Monitor the radiation safety program, and investigate spills and threats of radiation exposure; May perform Radiation Safety Officer duties as assigned by Pharmacy Manager or in Manager’s absence where appropriate;

  • Provide quality control of products and equipment; Ensure quality checks and maintenance are conducted timely and accurately; Peak single-channel analyzer and adjust for maximum efficiency;

  • Counsel patients about prescriptions, respond to inquiries and resolve problems; Research new formulas for patients, and determine the method of dispensing; Communicate to patient on how to use the drugs, and what the expected results may be;

  • Manage inventory, order supplies as necessary to maintain adequate volume to ensure operational efficiency of the facility; May be required to provide rotational coverage to other pharmacy locations throughout the region;

  • Supervise pharmacy technicians and support team; Assign and check work, assist in performance evaluation, discipline, and recommend hire and/or termination;

  • May perform the duties and functions of pharmacy technicians and support team; Operate vehicle to deliver products to hospitals, clinics, and other facilities as instructed; Assist in delivery scheduling and vehicle routing; Respond to inquiries pertaining to deliveries and routing;

  • Collect and return used materials and instruments; Check returns for contamination and dispose of properly; Disassemble syringes, dispose of needles into an approved container, remove a label from pig and test for contamination; Restock clean returns and instruments into inventory;

  • May serve in Quality Manager and/or Cyclotron Operator III role for PET pharmacy operations, as applicable; Refer to Role Description for Quality Manager and/or Cyclotron Operator III for additional duties and responsibilities;

  • May also serve as Radiation Safety Officer (RSO) for the pharmacy, as applicable; Refer to the Role Description for RSO for additional duties and responsibilities;

  • Perform or assist with any operations, as required to maintain workflow and to meet schedules and quality requirements; Notify supervision of unusual equipment or operating problems and the need for additional material and supplies;

  • Maintain a safe work area and comply with safety procedures and equipment operating rules keeping work area in a clean and orderly condition;

  • Participate in any variety of meetings and workgroups to integrate activities, communicate issues, obtain approvals, resolve problems and maintain a specified level of knowledge pertaining to new developments, requirements, policies, and regulatory guidelines;

REQUIREMENTS:



  • Bachelors of Science degree in pharmacy or a Doctor of Pharmacy from an accredited college of pharmacy required;

  • “Authorized User” of radioactive materials on the pharmacy’s radioactive materials license and active state licensure preferred;

Minimum Required Qualifications

Active Pharmacy License in the state in which you are employed

Not on the DEA Excluded Parties List

Immunization Certification through an accredited organization (e.g., APhA)

No pending felony charges or convictions for criminal offenses involving controlled substances
