The Clinic Nurse (RN) will plan, coordinate, and oversee our day-to-day nursing services. They will support our primary care providers in patient adherence to treatment goals through coordination of care. In addition to clinical support, the RN will help us achieve our administrative goals. Opportunities will be available for professional growth and development through participation in educational programs, review of current literature, in-service meetings, and workshops.    

● Performs timely client/patient telephone advice and triage assessment with clinical competence and judgment for urgent/drop in care; 

● Documents appropriately in electronic medical record; 

● Uses multiple EMR platforms to gather patient information; 

● Responds to emergencies as they occur in a timely fashion with clear delegation to clinic staff; 

● Performs nursing assessment, implements plan of care, and arranges for follow up care for primary care patients; 

● Delegates MA tasks to enhance patient care and clinic flow, and mentors MA staff on competencies 

● Provides patient education on a variety of topics (e.g. HIV, diabetes, hypertension, asthma, nutrition, and other specific diagnostic procedures); 

● Provides team-based medical management according to defined algorithms for patients with chronic diseases; 

● Provides wound care service under supervision of providers and per clinical protocols on site and as part of our street medicine team. 

● Participates in daily Quality Improvement activities; 

● Performs initial and continued assessment of client health status, immunizations, and TB screening based on established protocols;

● Administers adult immunizations, medications, and phlebotomy consistent with current guidelines and established protocols; 

● Provides culturally and racially competent client care in a manner that is both non-discriminatory and non-judgmental; 

● Communicates effectively with client/patient, clinic staff, the public at large and departmental staff in a non-discriminatory manner; 

● Works with other members of the health care team to provide comprehensive primary care services; 

● Participates in pre-clinic daily huddle, identifies high-needs patients for the clinic session; 

● Supervises and trains non-licensed staff working in the clinic (e.g. Medical Assistants) when assigned by clinic management; 

● Covers clinical job duties of non-licensed staff when required by clinic management; 

● Conducts intake appointments for new patients. Proficiently triages patients based on acuity, complexity, and immediate needs to appropriate Primary Care Provider;  

● Links patient to appropriate in-house and outside medical, behavioral, and social services; 

● Supervises vaccine management program in conjunction with Department of Public Health; 

● Conducts and / or supervises administrative aspects of care coordination (eg. prior authorizations, referrals, post-hospitalization discharge); 

● Conducts and/ or supervises medication and vaccines inventory as well as crash cart inventory. 

● Evaluates and executes routine refills and helps navigating prior authorizations according to established work flows; 

● Coordinates treatment plans/procedures for routine services such as pre-exposure prophylaxis (PrEP), drug detox, and STI treatment; 

● Coordinates follow-up care in accordance with treatment plan outlined by Primary Care Provider and interdisciplinary team; 

● Evaluates patients for medication adherence per Primary Care Provider’s treatment plan. Conducts accurate and thorough medication reconciliation; 

● Serves as integral member of the clinical and interdisciplinary team to provide wrap-around services/care for clients in a team-based care model; 

● Work may be split between our two clinic sites and street medicine. Any travel required to fulfill job duties will be reimbursed; 

● Other job-related duties as assigned.     

Minimum Qualifications:  

● Bachelor's degree in Nursing; at least 3 years of experience that can be demonstrated to be applicable to the duties listed in the job description. OR Associate's degree in Nursing; at least 5 years of experience that can be demonstrated to be applicable to the duties listed in the job description.  

● State of California Licensed Registered Nurse or licensure pending as documented by temporary licensure. 

● Maintains current BLS certification. 

● Effective written and verbal communication skills. 

● Must have a philosophy that is consistent with the Mission, Vision, and Core Values of the organization. 

● Ability to organize, plan, and effectively work on an interdisciplinary team.   

Preferred Qualifications:  

● Experience working with people of diverse backgrounds (gender identity, sexual orientation, race, ethnicity, immigration status). 

● Experience working with clients affected by significant challenges with behavioral and/or mental health. 

● Experience applying harm reduction principles in clinical care. 

● Experience working with highly-traumatized populations, using trauma-informed principles of care. 

● Fluency in English and an additional language (e.g., Spanish, Thai, Laotian, Vietnamese or Chinese).

Americare Hospice & Palliative Care prides itself in providing quality patient care for the past 15 years. We are seeking Registered Nurses to fill 3 positions: FT, PT, and On Call. The full time position for RN Case Manager is for Monday through Friday 9a-530p for care of hospice patients. No on call responsibilities are given to full time RN Case Managers. The part time position is for a PRN Admission Nurse at a per visit rate of $200/visit. The on call position is for every other weekend nights (Fri night, Sat night, Sun night) paid at $525 per weekend on call plus additional pay for visits ($1050 for 2 weekend nights a month at minimum, plus pay for visits).


Seeking candidates who are competent with nursing skills, are sensitive with grieving patients and families, are a team player, and are able to adapt and keep up with a fast-paced environment. Must have an RN License, hospice experience preferred, EHR used: MUMMS.


The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.


Responsibilities:


Patient Care


1. Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).


2. Provides professional nursing care by utilizing all elements of nursing process.


3. Assesses and evaluates patient’s status by:


A. Writing and initiating plan of care


B. Regularly re-evaluating patient and family/caregiver needs


C. Participating in revising the plan of care as necessary


4. Uses health assessment data to determine nursing diagnosis.


5. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.


6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.


7. Counsels the patient and family in meeting nursing and related needs.


8. Provides health care instructions to the patient as appropriate per assessment and plan.


9. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.


10. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.


Communication


1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.


2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.


3. Communicates with community health related persons to coordinate the care plan.


4. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.


5. Provides and maintains a safe environment for the patient.


6. Assists the patient and family/caregiver and other team members in providing continuity of care.


7. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.


8. Attends interdisciplinary group meetings.


Additional Duties


1. Ensures that arrangements for equipment and other necessary items and services
are available.


2. Supervises ancillary personnel and delegates responsibilities when required.


3. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.


4. Fulfills the obligation of requested and/or accepted case assignments.


5. Actively participates in quality assessment performance improvement teams and activities.


Qualifications:


1. Registered nurse with current licensure to practice professional nursing in the state.


2. Graduate of National League for Nursing accredited school of nursing.


3. Maintains a current CPR certification.


4. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.


5. Minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.


6. Self directed and able to work with minimal supervision.


7. Management experience not required. Responsible for supervising hospice aides.


8. Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.


9. Shows ability to organize and prioritize workload independently.


10. Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.

We are seeking a PRN (on-call) Registered Nurse (RN) to assume responsibility and accountability for the management of nursing and home health functions as well as the utilization of ancillary agency staff. Summary:


The On-Call Nurse is responsible for monitoring the health and well-being of participants in the Adult Day Service (ADS) Program and for assisting the ADS Manager with the daily operations of the program.


Essential Functions:


Operational



  • Monitor the health of participants. Respond to questions, complaints, and symptoms; respond to illnesses and/ or emergencies

  • Assist the Manager in the day-to-day operation.

  • Administer and record daily medications as prescribed by the participant's physician.

  • Monitors vital signs and weights as required by regulations. 

  • Observe and document changes in participant functioning level and confer with case manager/family regarding any issues.

  • May assist with maintaining current physical, care plan and quarterly review on each participant as required by regulations as assigned.

  • Supervise the service of meals and snacks and maintain required documentation.

  • Assure nutritional needs of participants are monitored and addressed.

  • Ensure that participants special needs are met by staff, such as ambulation hydration, dressing changes, toileting and personal hygiene.  Provide personal care assistance when needed. 

  • Report any incident of abuse or neglect as mandated by state guidelines.


Organizational:



  • Attend staff meetings, training, and seminars as required.

  • Must comply with all Senior Friendship Centers personnel policies.

  • Must protect the safety and health of staff, volunteers, participants, and others by observing and following the organization’s safety and incident policies.

Supervisory Responsibilities:


  • None.

Education, Experience, Licenses, and Certifications:



  • Requires a current Florida nursing license, minimum requirement is LPN.

  • Basic Life Support (BLS) certification.

  • Proficient with Microsoft Office Suite (Word, Excel, Outlook). Must have experience using computer software applications for record management and the ability to quickly learn the organization’s systems.


Physical Demands:



  • Prolonged periods sitting at a desk, working on a computer, and standing.

  • Must be able to lift 30 pounds.

  • Must be able to navigate various departments of a physical location.

  • Must be able to assist potentially frail seniors in wheelchairs or using assistive devices.

  • A valid driver’s license and proof of insurance are required for all work-related travel.        


Additional eligibility requirements:



  • Satisfactory clearance of a fingerprint-based, level II background screening, tuberculous test, and pre-employment substance abuse testing is required. 

  • Must be authorized to work in the United States.
