The role:

The most important function of the Facility Manager’s role is to ensure that staff provides 24/7 coverage, as the safety of all adolescents in care is our number one priority. In this role, you will assist the Clinical Program Director in the supervision of staff that provide direct child care services to adolescents in treatment; our objective is to make certain that all needs and services for each client are being implemented. In this position, you will be not only the Manger of the facility but also the lead Nurse. This postion is about 70% Facility Management duties, and 30% Nursing duties.

Administrative duties:


  • Work alongside and report directly to Clinical Program Director

  • Direct supervisor to Weekend LVN and Residential Counselors

  • Warmly welcome guests; coordinate and schedule facility tours for new clients

  • Create weekly staff schedule

  • Coordinate program and client schedules

  • Demonstrate model Human Resources standards and maintain personnel files

  • Coordinate payroll related issues with Human Resources department

  • Ensure facility compliance with accreditation and program standards

  • Responsible for the patient discharge process and satisfaction surveys

  • Ensure facility grounds are well kept

  • Responsible for the management of facility finances

  • Leader of safety protocols, including the stocking of emergency food supplies, conducting checks and tests (I.e.: for smoke detectors and fire extinguishers), and that all emergency binders are current and updated

Nursing duties:


  • Medication inventory and daily medication administration

  • Conduct weekly urine and analysis and send to labs

  • Coordinate with psychiatrist weekly on medication changes and refills

  • Provide first aid and nursing care as needed

  • Record daily vitals and weekly weight

  • Conduct Nursing Assessment for each client at admission

  • Liaise between clients and guardians regarding medication

  • Ensure all client records are up to date, request information when needed, and arrange appointments for each client as needed

Hours: Monday through Friday, 9:00am-5:00pm

Availability on weekends and evenings for emergencies

Basic Qualifications:

  • CRITICAL CARE - One year recent (within the last 3 years) experience in Critical Care. Current ACLS certification required. Completion of a Critical Care course preferred.


  • SPECIALITY AREAS - If no qualified candidate, may be willing to train in the specialty areas listed below. Candidates in training are required to obtain appropriate certifications within 6 months of hire or transfer into unit.


  • High School Diploma/GED.


  • Graduate of an accredited registered nursing program and completion of the current hours required by the BRN of RN student clinical experience or Graduate of registered nursing and either 1) 2000 hours of RN experience in clinical area of specialty from a licensed acute care facility within three (3) years of graduation; or 2) graduate of an accredited licensed vocational nursing program that includes clinical student experience in acute care and has worked 2000 hours as an licensed vocational nurse within the clinical area of specialty within three (3) years before graduation or currently enrolled in an accredited entry level MSN program and has completed pre-licensure requirements to practice nursing.


  • Current license to practice as a Registered Nurse in California.


  • Current Basic Life Support (BLS) certification for health care providers.


  • Demonstrated knowledge of the RN scope of practice.


  • Demonstrated commitment to service orientation (members, staff, providers).

Nurse must enter community if called in within one hour of initial notification.

Any failure to meet responsibilities #-4 or #-5 will count as a call off for Unit Manager and will accumulate points according to Attendance Policy.

Nurse is to oversee assessments of assigned group and to assist in the requirements needed by ODH regulations as directed by supervisor.

Nurse is to hold a unit specific activity once monthly.

Essential Duties and Responsibilities:

  • Be cross-trained in other roles of the agency to be able to assist as needed and directed.

Compensation includes: 

Friday night 5pm - 12 midnight on call ($50.00)

Saturday and Sunday whole day and evening on call ($75.00 each day)

If you are given a case and make a visit to a client's home, you'll be compensated $60.00 per hour.  The above rates for on call are strictly to be available for any and all cases that arise.

  • Admissions/Evaluations as ordered during weekend hours as well as obtaining history/physical at the time of admission or begin records requisition process.

  • Call the patient/family/caregiver


  • Visiting the patient, if necessary and/or sending information to primary on call nurse


  • Obtaining physician (or other authorized independent practitioners) orders, as needed


  • Arrange for other hospice services, as needed


  • Document each patient/family interaction in a clinical note


  • Maintain an on-call log of all patient contacts during on-call hours


  • On-Call staff will respond to a page within 15 minutes and must be able to reach a patient within one (1) hour. There may be rare exceptions, depending on how far away the patient lives and if the staff member is with another patient at the time of the page


  • Reports will be given to the on-call nurse before their shift begins, preferably using the report line or verbal communications


  • The on-call nurse will report his/her Weekend patient care activities to the RNCM, DOC, and Administrator


  • Provides professional nursing care by utilizing all elements of the nursing process.


  • Counsels the patient and family in meeting nursing and related needs


  • Provides health care instructions to the patient as appropriate per assessment and plan of care


  • Facilitate the patient's efforts toward optional comfort care

SUMMARY OF JOB FUNCTION



Under the supervision of the GFHN Director of Nursing for Nursing Protocol, and direct supervision of the Clinic Coordinator for daily operations is responsible for overseeing services as it relates to nursing care. This individual will work within the scope of nursing practice and licensure from the State of California in accordance with Title 22 and the guidelines of patient Centered Medical Home Model. As a team member the Registered Nurse collaborated with clinic management, clinician staff, and all clinical support staff to maintain standard of care with the patient at the center. RN is directly involved in the process of cursory assessment of the patient, advising the patient, and determining disposition of the patient according to patient’s urgent healthcare need.


 


ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:


To perform the job successfully, an individual should demonstrate the following competencies:
 


1.      Via phone assess and evaluates patient’s urgent level of care needed, assist patient determine if they need emergency treatment, make an appointment, or advise on treating themselves at home.


2.      Ability to assist as back up with hands on patient care as it relates to nursing process.


3.      Document in the Practice Management System all nursing phone/in person nurse advice, in-house clinician consultation, and patient visit outcome.


4.      Interacts with healthcare team, hospital emergency staff, and /or specialty physician consultations, as needed.


5.      Follow advise nursing policies and procedures as related to patient’s urgent healthcare need, uses GFHN nursing triage book as a guideline in determining patient’s disposition.


6.      Working under GFHN nursing protocols, standing orders, becomes familiar with clinical support staff job description and daily expectations to meet patient nursing needs.


7.      Provide health education and counseling to the patient and family when indicated.


8.      RN will dispense medication using GFHN nursing protocols, as needed, monitoring meds for expiration, and following GFHN drug disposal policy


9.      RN will work closely with GFHN referrals department, as needed for patient’s continuous healthcare follow-up.


10.  RN will submit any reports or statistics related to triage department, as requested by DON, or GFHN administrative staff.


11.  In a Code Blue RN will participate in triage/assess patient, staff, or visitor, have staff call 911.


12.  Assures all nursing care provided is documented on date services are given.


13.  Assures health education and counseling to patient and family are provided as indicated by the plan of care


14.  The RN is hired for GFHN and may be asked to be reassigned from one clinic to another as the need arises.


15.  Responsible for keeping the work area in a clean, safe, and orderly manner.


16.  Responsible for following facility fire safety, infection control and hazardous chemical procedures and for promptly reporting any unsafe conditions or equipment problems.


17.  Working with Clinic Coordinator for ordering an appropriate amount of supplies and maintaining adequate inventory for appropriate nursing equipment and supplies she/he uses on a daily basis.
