The role:

The most important function of the Facility Manager’s role is to ensure that staff provides 24/7 coverage, as the safety of all adolescents in care is our number one priority. In this role, you will assist the Clinical Program Director in the supervision of staff that provide direct child care services to adolescents in treatment; our objective is to make certain that all needs and services for each client are being implemented. In this position, you will be not only the Manger of the facility but also the lead Nurse. This postion is about 70% Facility Management duties, and 30% Nursing duties.

Administrative duties:


  • Work alongside and report directly to Clinical Program Director

  • Direct supervisor to Weekend LVN and Residential Counselors

  • Warmly welcome guests; coordinate and schedule facility tours for new clients

  • Create weekly staff schedule

  • Coordinate program and client schedules

  • Demonstrate model Human Resources standards and maintain personnel files

  • Coordinate payroll related issues with Human Resources department

  • Ensure facility compliance with accreditation and program standards

  • Responsible for the patient discharge process and satisfaction surveys

  • Ensure facility grounds are well kept

  • Responsible for the management of facility finances

  • Leader of safety protocols, including the stocking of emergency food supplies, conducting checks and tests (I.e.: for smoke detectors and fire extinguishers), and that all emergency binders are current and updated

Nursing duties:


  • Medication inventory and daily medication administration

  • Conduct weekly urine and analysis and send to labs

  • Coordinate with psychiatrist weekly on medication changes and refills

  • Provide first aid and nursing care as needed

  • Record daily vitals and weekly weight

  • Conduct Nursing Assessment for each client at admission

  • Liaise between clients and guardians regarding medication

  • Ensure all client records are up to date, request information when needed, and arrange appointments for each client as needed

Hours: Monday through Friday, 9:00am-5:00pm

Availability on weekends and evenings for emergencies

Job Summary

Clinical Mid-Level Provider, with the supervision of a physician, provides a continuum of care to the chronic pain patient. The Mid-level provider performs diagnostic and therapeutic interventions to manage acute and chronic pain focused on a full range of treatment options and avoidance of drug dependencies.

Job Responsibilities

Conducts thorough medical histories, performs complete physical examinations, and initiates appropriate lab and radiology testing required for evaluation of illness.

Assess the patient’s current health status and develops a medical diagnosis and treatment plan, with a direct review of a physician.

Prescribe appropriate oral, transdermal and Intrathecal medications. Counsels patients on drug side effects and hazards and concurrent use of alcohol and other medications.

Performs routine procedure therapeutic procedures we train on including trigger point injections, ITMSrefills and reprogramming, spinal stimulation analysis and reprogramming, ITMS aspiration and evaluation of peripheral nerve injections and surgical wound care.

Educates patients regarding their disease and treatment. Counsels on preventive health maintenance topics such as diet, weight, and smoking.

Assists with record keeping and completes appropriate medical charts.

Identifies potential candidates for clinical research trials and coordinates with the clinical research coordinator on presenting options to patients.

Refers patients to physicians or to specialized health resources, as appropriate.

Demonstrates a high standard of moral and ethical behavior. Demonstrates compassion and professionalism and a commitment to outstanding patient care.

General Purpose

The primary purpose of your job position is to assist the Nurse Supervisor/Charge Nurse in their day-to-day recording and charting of medical or administrative information in accordance with current federal, state, and local standards, guidelines, and regulations that govern medical records, and as may be required by the Director of Nursing Services, to ensure that our medical records are maintained in an informative and descriptive manner.

Essential Duties

* Record medical and administrative information in accordance with our established charting and documentation policies and procedures

* Maintain the Daily Census Report and submit to Nurse Supervisor/Charge Nurse.

* Maintain an up-to-date roster of residents for your assigned unit.

* Fill out resident charge slips and submit to the Business Office.

* Answer telephone, page calls, deliver messages to residents, etc., as necessary.

* Maintain a current listing of emergency phone numbers for your assigned unit.

* Maintain and forward daily tardy and absentee reports to the Business Office

* Record appropriate resident identification data on designated medical records, wristbands, ID cards, etc., as required.

* Transcribe physicians’ orders to care plans, medication cards, treatment plans, etc., as required.

* Report to the Nurse Supervisor/Charge Nurse discrepancies found in transcribing physicians’ orders, diet orders/changes, charting, etc.

* Review medication cards for completeness of information, accuracy in the transcription of the physician’s order, legibility, etc., as directed.

* Chart nurses’ notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care.

* Forward new diet orders and/or diet changes to the Director of Food Services.

* Notify the resident’s next-of-kin when there is a change in the resident’s condition as instructed.

* Admit, transfer, and discharge residents. Assist in arranging for transportation, packing residents’ belongings, escorting them to discharge/transfer area, loading, etc., as necessary.

* Complete necessary medical and administrative records upon the resident’s admission, transfer, and/or discharge.

* Forward completed charts of discharged residents to the Medical Records/Health Information Department.

* Attend various committee meetings of the facility (i.e., Infection Control, Policy Advisory, Pharmaceutical, Quality Assurance & Assessment, etc.) as required.

* Assist the Nurse Supervisor/Charge Nurse in developing work assignments, schedules, etc., as required.

* Develop and maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility.

* Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, etc.

* Greet newly admitted residents upon admission. Escort to room as necessary.

* Encourage attending physicians to record and sign progress notes, physicians’ orders, etc., on a timely basis and in accordance with current regulations

* Inform family members of the death of a resident as instructed.

* Call funeral homes when requested by the family.

* Attend and participate in annual facility in-service training programs as scheduled (e.g., OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.).

* Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.

* Participate in the implementation, and maintenance of the infection control program for monitoring communicable and/or infectious diseases, including TB, among the residents and personnel.

* Inform the Nurse Supervisor/Charge Nurse of your equipment and supply needs.

* Ensure that all personnel involved in providing care to the resident are aware of the resident’s care plan and that nursing personnel refer to the resident’s care plan prior to administering daily care to the resident

* Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc.

* Treat all residents fairly, and with kindness, dignity, and respect. * *

Assist in recording all incidents/accidents. File in accordance with established policies and procedures.

* Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.

      Jerome Home is a 120 bed Long Term Care Facility in New Britain, CT. Established under the will executed by Augustus Jerome, Jerome Home is a privately-owned, not-for-profit organization. We are a progressive, nursing community with a highly competent and caring staff. Established in 1932, our multi levels include; 26 units on RCH (Resident Care Home), 2 – 32 bed neighborhoods on SNF, and 1 – 30 bed rehab/SNF. Also on our campus is Arbor Rose, a 62 apartment Assisted Living Facility.


      Our part time positions are on the 3-11 shift. A few are available on 11-7. We also are hiring for per diem all shifts


      We take pride in our patient- centered care as our residents always come first. We have a dedicated senior population (no one under 60 can be admitted) except for our rehab neighborhood. Those residents could be under age 60, but will be discharge to home once rehab is completed.


      As an RN Charge Nurse, you will work with a professional team of CNA’s to provide our residents the care and services they need to thrive. All of our nurse are IV certified. On evenings, our CNA ratio is excellent – up to a 6:1 ration on the 3 long term care neighborhoods.
