Job Summary
Clinical Mid-Level Provider, with the supervision of a physician, provides a continuum of care to the chronic pain patient. The Mid-level provider performs diagnostic and therapeutic interventions to manage acute and chronic pain focused on a full range of treatment options and avoidance of drug dependencies.
Job Responsibilities
Conducts thorough medical histories, performs complete physical examinations, and initiates appropriate lab and radiology testing required for evaluation of illness.
Assess the patient’s current health status and develops a medical diagnosis and treatment plan, with a direct review of a physician.
Prescribe appropriate oral, transdermal and Intrathecal medications. Counsels patients on drug side effects and hazards and concurrent use of alcohol and other medications.
Performs routine procedure therapeutic procedures we train on including trigger point injections, ITMSrefills and reprogramming, spinal stimulation analysis and reprogramming, ITMS aspiration and evaluation of peripheral nerve injections and surgical wound care.
Educates patients regarding their disease and treatment. Counsels on preventive health maintenance topics such as diet, weight, and smoking.
Assists with record keeping and completes appropriate medical charts.
Identifies potential candidates for clinical research trials and coordinates with the clinical research coordinator on presenting options to patients.
Refers patients to physicians or to specialized health resources, as appropriate.

General Purpose

In General, the duties of the Position [RN, LPN, CNA, Home Health Aide or Companion] to be filled by the Temp shall encompass, but not limited to the following:
1. LPN & RN
· Provide direct care, support, and comfort to WonderStar clients/patients and family members at both at licensed healthcare facilities and homes.
· Assess and admit patients to licensed healthcare facilities when required by WonderStar’s contractual agreement consistent with home health care regulations and requirements as well as the patient acceptance of policies as provided, i.e. like those involving MFP-Service’s Contract.
· Make skilled intermittent home visits consistent with physician's orders and
company’s contract maintenance procedures.
· Provide private duty shift nursing consistent with both physician's orders and WonderStar’s contractual agreement.
· Coordinates care plan development with WonderStar clinical management and physicians.
· Secure and implement interim orders from physicians as needed.
· Maintain people clinical notes and documentation consistent with good nursing
practice and NCS policies.
· Maintain appropriate communication with physicians and coworkers regarding
the patients care.
· Maintain open and direct communication with the patient and family members at
all times.
· Demonstrates clinical knowledge and judgment in the treatment of a wide range
of medical conditions.
· Prepares Oasis assessment form, 485's and other required home healthcare
documents and forms as may be required.
· Participates in quality improvement and utilization review processes as requested.
· Demonstrates knowledge in G-tube, trach, and vent care method practices and
procedures.
· Demonstrates competency and experience in the use of infusion pumps and
therapy.
· Demonstrates a caring and compassionate demeanor at all times.
· Functions with a high degree of independence requiring minimal supervision.
2. CNAs including HOME HEALTH AIDES
· Answer patients' call signals, help patients to eat, dress and bathe, deliver messages, serve meals, turn and reposition bedridden patients to prevent bedsores
· Observe patients' conditions, measure and record food and liquid intake, listen to and record patients’ health concerns and report that information, measure patients’ vital signs, such as blood pressure and temperature
· Provide patients with help walking, exercising, and moving in and out of bed, provide patient care by applying dressings and supervising exercise routines, prepare patients for surgery, treatment, or examination.
· Transport patients to treatment units, using a wheelchair or stretcher, clean rooms and change linens.
3. CORE SERVICES & COMPANION SERVICES
· vacuuming, washing floors and walls, defrosting freezers, cleaning ovens, cleaning attics and basements to remove fire and health hazards, changing storm windows, performing, heavy yard work, shoveling snow and making minor home repairs.
· Ability to perform or carry out all tasks described specifically under MFP – Chores Services
· Supervision, socialization, and non-medical care to a functionally impaired adult.
· Assistance or help in supervising the individual with such tasks as meal preparation, laundry and shopping, but do not perform these activities as discrete services.
· House cleaning and maintenance, including vacuuming, dusting, dry mopping, dishwashing, cleaning the kitchen/bathroom and changing beds.
· All services provided in conformity with a therapeutic goal that may be set in the plan of care.
