Currently, CareMount Medical is looking to grow our collaborative practice is nestled in the beautiful Hudson Valley of New York State along the Hudson River. This area of New York is rich with history, recreation, excellent schools and is an hour and a half from New York City. The practice is affiliated with Northern Dutchess Hospital in Rhinebeck. Births are attended at the Neugarten Birth Center within the hospital with excellent nursing and physician support for low tech birth.


The Northern OB/GYN department of CareMount Medical consists of 3 obstetrician/ gynecologists, 2 midwives, one gynecologist and a physician assistant. Midwives are responsible for both OB and GYN care of clients in offices in Rhinebeck and Kingston.


Position Overview: The Certified Midwife provides care and treatment to women throughout their lifetime including sexual health and contraception; prenatal, birth and postnatal health; and gynecologic health from adolescent through menopause.


Essential Duties and Responsibilities (including but not limited to the following):
· Participates in initial examination of obstetrical patient, and in collaboration with the MD, shares responsibility for care, treatment, and delivery of patient.
· Examines patient during pregnancy, utilizing physical findings, laboratory test results and patient’s statements to evaluate condition and ensure that patient’s progress is normal.
· Discusses case with Obstetrician to assure observation of specified practices.
· Instructs patient in diet and prenatal health practices.
· Stays with patient during labor to reassure patient and to administer medication.
· Delivers infant and performs postpartum examinations and treatments to ensure that patient and infant are responding normally.
· When deviations from standard are encountered during pregnancy or delivery, administers stipulated emergency measures, and arranges for immediate contact of Obstetrician.
· Visits patient during postpartum period in hospital to instruct patient in care of self and infant and examine patient.
· Maintains records of cases for inclusion in established medical file.
· Conducts classes for groups of patients and families to provide information concerning pregnancy, childbirth, and family orientation.


Performance Requirements:
· Knowledge of CareMount Medical policies, regulations, and procedures to administer patient care.
· Adheres to all CareMount Medical’s policies and procedures including OSHA and HIPAA guidelines.
· Knowledge of medical equipment and instruments to administer patient care.
· Knowledge of common safety hazards and precautions to establish a safe work environment.
· Skill in applying and modifying the principles, methods, and techniques to provide on-going patient care.
· Skill in developing and maintaining department quality assurance.
· Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to maintain quality control standards.
· Ability to react calmly and effectively in emergency situations.
· Ability to interpret, adapt, and apply guidelines, and procedures and the ability to communicate clearly.


Qualifications and Education:
• Master’s degree and Midwifery certificate
• Completion of Midwifery, Completion of Midwifery Education Program
• Four years of clinical Midwifery experience, required with references
• Current NYS Midwife License and AMCB certification
• Working knowledge of NextGen Practice Management preferred


TYPICAL PHYSICAL DEMAND: Requires full range of body motion including moving and lifting patients, manual and finger dexterity and eye-hand coordination. Requires sitting and/or standing for extended periods of time. Occasionally exerts up to 100 pounds of force, such as assisting patients in arising from the exam table, pushing patients in wheelchairs, or assisting patients into/out of wheelchairs. Requires corrected vision and hearing to normal range.


TYPICAL WORKING CONDITIONS: May require frequent exposure to communicable diseases and body fluids, toxic substances, medicinal preparations, and other substances and conditions common to a medical office/healthcare environment. Requires use of telephone and computer, potentially for long periods of time. Requires driving to hospitals and CareMount Medical locations other than their primary CareMount office location. Provides midwifery care for patients with obstetric and gynecologic needs during labor delivery and postpartum. Identifies additional needs for mothers and babies in birthing units.

Monitors patients' condition during pregnancies, including co-management of high-risk pregnancies, and performs examinations and procedures. Monitor fetal development by listening to fetal heartbeat, taking external uterine measurements, identifying fetal position, or estimating fetal size and weight. Maintain and record information in medical records or provide narrative summaries to communicate patient information to other health care providers. Provides guidance to expectant mothers in prenatal self-care, e.g. nutrition, weight gain, and daily activity levels. Midwifery as practiced by certified nurse-midwives (CNM) and certified midwives (CM) encompasses a full range of primary health care services for women from adolescence beyond menopause. These services include primary care, gynecologic and family planning services, preconception care, care during pregnancy, childbirth and the postpartum period, care of the normal newborn during the first 28 days of life, and treatment of male partners for sexually transmitted infections. Midwives provide initial and ongoing comprehensive assessment, diagnosis and treatment. Mission Statement

Responsibilities*

  • To provide specialized care to essentially healthy women during pregnancy, labor, birth and postpartum, as well as care to normal women for well-woman gynecology, family planning and treatment of minor gynecological problems.
  • To provide support for midwifery service patients who are clinically managed by physician staff (eg cesarean births, breech versions, pregnancy terminations and preterm inductions for intrauterine fetal demise)
  • To practice as primary health care provider with consultation and collaboration with Obstetricians/Gynecologists if a medical problem arises.
  • To exercise leadership through clinical practice, student and staff education and research.

Organizational Relationships:

  • Reports to the Director of Nurse-Midwifery Service
  • Collaborative/consultant relationship with faculty physicians and senior House Officers in the Department of Obstetrics and Gynecology related to medical aspects of nurse-midwifery practice.


Associate staff nurse-midwives are non-bargained for, hourly employees who work to fill in during non-scheduled and scheduled paid time off.  Therefore, a consistent schedule each month is not guaranteed.  
Associate staff will be paid as following:

  • Straight time for hours worked.
  • Time and a half for hours worked during holidays.
  • No benefits, including professional development money. (Staff may request money for specific educational needs – this will be given per service director’s ability to grant this request).
  • Paid hour for hour for mandatory training requirements.
  • Paid hour for hour for time spent at staff meetings.
  • UMHS to pay for mandatory requirements such as BLS,  
  • Associate staff are required to possess the same skills as regular staff Midwives
  • Associate staff may be asked to work in the outpatient clinics, inpatient call and triage.
  • Each associate staff person will be required to work at least 12 scheduled hours during the month and not more than a full time staff person.  At times, the service will not need this much coverage from the associate staff.  The service director will take the following into consideration when assigning coverage
    • Seniority
    • Availability to fill the service need.
    • Match the need to the skills of the person.
  • Associate staff are also expected to share with regular staff members, the burden of covering the service when unexpected emergencies occur that affects staffing patterns 
  • Associate staff are willing to be contacted in the event of unexpected emergencies.
  • Other requirements of Associate Staff 

  1. Attend monthly staff meetings.
  2. Attend Birth Options if taking more than 3 calls/schedule (those just doing clinic do not need to attend options)
  3. Attend staff retreats.
  4. Attendance at departmental events in accordance with the expectations of the Ob/Gyn Department to maintain credentialing.
  5. Complete mandatories, employee health requirements and participate in yearly evaluations.  
  6. Participate in training that are required to provide patient care UMHS such as computer training.

Other exceptions to regular staff requirements.

  1. No holiday requirements.
  2. Do not need to bid for PTO.
  3. Are not expected to take back-up weekend call.
  4. Are not expected to give the Birth Options presentation.

End of Employment

  • Associate staff is expected to give 8 weeks notice when terminating employment.
  • Service Director is expected to give 8 weeks notice when their services are not longer consistent with the needs of the service.  
  • If termination is due to disciplinary reasons or documentation of poor clinical performance, no notice is required.

Extensive Resection of Endometriosis off of colon, and Bilateral Ureterolysis to completely rid of endometriosis.) We are one of very few gynecological surgeons capable to perform such procedures in the entire NY metropolitan area. We are also fully trained to operate the Robot in order to offer a full range of services in minimally invasive surgery for women in all stages of their lives. We are also known for our success in complicated obstetrical & gynecological surgeries. For instance, many women having the diagnosis of multiple cesarean sections or placenta previa/acreta/percreta are told by their obstetricians to seek their care elsewhere. Taking care of a woman with her 10th cesarean or a very complicated placenta acreta/percreta is a common practice for him.

Requirements



  • Master of Science degree in Nursing or DNP required.

  • Successful completion and graduation from an accredited school of Nurse-Midwifery preferred.

  • Current RN license required.

  • Current national certification as a Nurse Midwife preferred.

  • Professional nursing experience in a clinic setting required, with previous experience in womens health, gynecology, obstetrics or nurse midwifery preferred.

  • C-Section/First Assist in surgery experience preferred.

  • Current CPR certification required.

  • Must be able to pass a background check and drug screen

Role Purpose

Responsible for assisting women in all stages of pregnancy, childbirth and post-delivery. Work is performed with considerable independence under the general medical direction of an Obstetrician/Gynecologist. Works is guided by physician-approved protocols and evaluated through chart review, consultations and observation of results.


Responsibilities

  • Monitor, counsel and educate patients to promote healthy living through education and primary prevention regarding medications, treatments, anticipated outcomes and referrals.
  • Conducts examinations and takes full detailed medical history to identify and assess patients clinical and health care needs to develop an individual program of care.
  • Identify high risk pregnancies and make referrals to doctors and other medical specialists, as appropriate.
  • Supervise and assist mothers in labor, monitoring the condition of the fetus and using knowledge of drugs and pain management
  • Offer support and advice following birth or events such as miscarriage, termination, stillbirth, neonatal abnormality and neonatal death
