 Clinical Supervisor for a small non-profit year-round Special Education School and Medi-Cal Mental Health Clinic.

Program Description:La Cheim's school programs provides both special education and mental health services to adolescent and latency age children. All students have an IEP issued from a neighboring public school district. Students receive educational and therapeutic clinical services in small, self-contained classrooms. Each classroom's treatment team consists of a special education teacher, clinician, mental health resources specialist, and classroom assistant. Additionally highly trained Psychiatrists, RNs, and NPs assist each treatment team in developing a quality, thorough, treatment plans for each child.

Each classroom's therapeutic milieu provides individual and group therapy for the participating students. Classroom's student sizes are small, averaging between 5 to 7 students. Overall campus student census ranges from 12 to 18 students. This allows our treatment plans to be very specific for each student's individualized clinical and educational treatment needs.

Our school provides year-round educational and mental health services. Family therapy is a key component to successful client treatment. A child cannot be successfully served unless their family, extended family, and other outside care givers are involved.

La Cheim has a very dedicated treatment team. Staff turnover is quite low; most employees have been with La Cheim well over 15 years. Duties Include:

• Overall responsibility for ensuring entire Program provides the most effective clinical and therapeutic services possible.

• Supervision of Program's clinical staff. Signing off hours for clinical interns.

• Oversee the completing Medi-Cal charting. Overseeing the clinician's Medi-Cal charting for accuracy and timeliness. Ensuring MD's, NP's and RN's, charting is complete and submitted in a timely manner.

• Ensuring all clinical notes have effective interventions to ensure each clients' clinical goals are best met.

•Ensure all employees successfully complete the maximum number of quality billable notes each month.

• Responsible for the entire Program's Mental Health/ Medi-Cal services including ensuring all records and files are kept in 'Audit Ready' condition, physical facility meets Medi-Cal standards, and client charts go to mandated county mental health reviews and audits.

Position Requirements/Qualifications:

• Must be a licensed MFT or MSW and be certified by the Board to supervise interns.

• Valid California Driver's License and a reliable vehicle.

• Current TB Test and clear criminal live scan clearances including DOJ & FBI.

• Past experience working with adolescents within a therapeutic milieu. (Preferred)

• Possess excellent administrative skills provided in an appropriate compassionate manner, yet be clear, firm, and direct.

PRINCIPAL SUBSTANCE USE COUNSELOR, FULL TIME SUBSTANCE USE TREATMENT OUTPATIENT PROGRAM POSITION DESCRIPTION AND JOB ANNOUNCEMENT

Union Position Horizons Unlimited of San Francisco, Inc. (Horizons), established in 1965, is a community-based organization located in the Mission District, and offers substance use prevention, treatment, employment, mental health and gender specific services, for Latino and other youth of color, ages 12 to 26, and their families, residing in the Mission District and the city and county of San Francisco.

Position Title: Principal Substance Use Counselor

Reports To: Executive Director (interim basis)

Program Summary : The Substance Use Outpatient Treatment Program (OP) provides culturally-rooted services that address the needs of youth in our community through individual, group, and family counseling, case management, collateral, and other healing-focused strategies such as art and music therapy, medicinal drumming, etc. Services are provided onsite, in the community including other CBOs, Juvenile Hall and San Francisco County Jails, at participating schools.

Responsibilities:

· Provide culturally and linguistically competent Substance Use Outpatient Treatment Services to youth and young adults ages 10-26.

· Work closely to with SFUSD and other City Department staff to coordinate referrals and service implementation to enhance a client’s experience and success in the program.

· Develop and implement an outreach plan and corresponding activities for the purpose of engaging youth in programming.

· Work collaboratively with Agency staff to increase knowledge and participation in OP services and initiatives, and support the facilitation of, and increase in, referrals and program support.

· Conduct client intakes, screening, and assessments to determine needs, strengths, supports, etc.

· Develop strength-based Plans of Care for each client and consistently monitor progress.

· Conduct individual, group, and family counseling, including follow-ups.

· Document and maintain up to date client files, progress notes, and plans of care while ensuring confidentiality, according to clinical procedures.

· Track client services and enter them into the AVATAR system on a daily basis, and within 24-48 of service provision.

· Act as an advocate for clients and families to ensure quality and responsive service delivery.

· Provide case management and collateral services that includes communicating regularly with schools, probation officers, social workers, family members and other caregivers, regarding client’s treatment progress, etc., when appropriate.

· Accumulate knowledge of, and coordinate services with other providers, when appropriate.

· Provide coordination support to achieve the mission and objectives of the Outpatient Program.

· Attend weekly Encuentro Clinical meetings, bi-weekly All Staff meetings and other funding source and required meetings.

· Attend all required trainings, including DMC ODS, documentation, other training's to stay abreast of program and service requirements.

· Participate in all relevant trainings (ASAM, DMC ODS, documentation, etc) to ensure knowledge and implementation of funding source requirement. · Perform other duties as requested by the Program Director, Treatment and Gender-Specific Programs.

· A minimum of 2.5 years working with at risk youth and their families.

· Must be a State Certified Counselor (or enrolled in an accredited institution and in the process of obtaining certification), through the CCAPP California Consortium of Addiction Programs and Professionals certification credentials in CADC, CATC, or CCAPP.

· Bilingual (Spanish/English) preferred.

· Knowledge in Motivational Interviewing, Stages of Change, Motivational Enhancement Therapy, Cognitive Behavioral Therapy, MET/CBT 5 treatment models.

· Knowledgeable of clinical treatment, healing arts, intervention techniques, and approaches to youth development. · Knowledge and ability to use Avatar system for all documentation.

· Experience working with youth within the juvenile justice system, youth with disabilities, youth with co-occurring disorders, and/or gang-affiliated, etc.

· Able to maintain confidential, accurate, and complete records including documentation of daily encounters; progress notes; weekly and monthly reports, etc.

· Must be a self-starter and able to organize/coordinate services/workflow in a flexible setting.

· Excellent organizational, communication, written, and verbal skills.

· Ability to work as a member of a team and work outside of regular business hours (that may include evenings, weekends, before and after hours).

· If in recovery, must be clean and sober for at least two continuous years.

This is a full-time, permanent position after successful completion of a 6-month probation period. This position is supervised by the Adult Services Clinical Director and/or designated clinical site supervisor or program manager. Unless specifically designated, this position does not supervise any other personnel.

Provides outpatient counseling, and other supportive covered services to eligible adult clients. Maintains client records and required paperwork. An acceptable level of productivity is expected to be maintained as measured by billable hours/service value.

  1. Provides brief, solution-focused individual, group and family therapy to a diverse caseload of adult clients, as evidenced by direct observation, treatment team meetings, and clinical record review.

  2. Maintains client records in compliance with Mohave Mental Health Clinic, Inc., State, HCIC and Joint Commission standards, as evidenced by clinical record reviews, peer review, and annual quality profile data. .

  3. Networks with community agencies to provide services to eligible clients, as evidenced by clinical record reviews, direct observation, and treatment team meetings

  4. Utilizes best practice guidelines to ensure appropriate length of time in care, as evidenced by review of assigned caseload, clinical record review and treatment team meetings.

  6. Provides after-hours and holiday crisis coverage on rotating schedule, as evidenced by review of on-call schedule and corresponding beeper logs.

  7. The employee must work well under pressure meeting multiple and sometimes conflicting deadlines. The employee shall, at all times, demonstrate cooperative behavior with colleagues and supervisors. This is evidenced through direct observation and interviews with co-workers and supervisors.

LINES OF AUTHORITY:


This position is supervised by a Children's Clinical Site Supervisor. Unless expressly identified, this position does not supervise any other staff.


POSITION SUMMARY:


Provides outpatient treatment for children and their families within the context of a system of care which includes collaboration with internal and external supports and resources.


PRINCIPAL DUTIES/RESPONSIBILITIES:



  1. Provides individual, group and family therapy in a variety of settings, to a diverse caseload of children and their families, as evidenced by direct observation, case staffings, and clinical record review.

  2. Develops with the client and family an integrated Behavioral Health Service Plan (BHSP) which identifies measurable goals and objectives developed by the child, family and their team and identifies the treatment and services to be provided, as evidenced by the BHSP, progress notes, case staffings and supervision.

  3. Assesses the client and family on a continuous basis, plans for discharge, monitors progress towards goals, and completes progress and case notes with every contact as evidenced by case staffings and the clinical record.

  4. Assists the family in identifying and utilizing formal and informal community supports with the assistance of other agency staff as evidenced by staffings with Family Involvement Specialists and Child/Family Support Staff and/or involvement in Child/Family teams.

  5. Develops clear, concise behavioral plans which utilize the Child/Family Support staff in providing training or support to the client and/ or the family as evidenced by behavioral plans and clinical supervision of Child/Family Support staff .

  6. Develops and maintains with the child and family team, a crisis safety plan as evidenced by the plan being in the clinical record.

  7. Maintains client records in compliance with Mohave Mental Health Clinic, Inc., State, HCIC and Joint Commission standards, as evidenced by clinical record reviews, peer review, and annual quality profile data.

  8. Adheres to current best practice standards to ensure appropriate length of time in care, as evidenced by review of assigned caseload, clinical record review and case staffings.

  9. Assists in crisis coverage and completion of pre-petition screenings/ evaluations when needed, as evidenced by direct observation and reports that are generated.

  10. Provides after-hours and holiday crisis coverage on rotating schedule, as evidenced by review of on-call schedule and corresponding beeper logs.

Job Description


Our Screening, Stabilization, and Transition (STAT) Program is an innovative, model program that provides mental health Screening, Stabilization and Transition (STAT) services to children as they enter the foster care system in Alameda County. The Foster Youth Mental Health Therapist completes a mental health screening and informal assessment of every child who arrives at the Assessment Center, providing both individual and milieu-based therapeutic interventions. In addition, therapists provide community-based transition services (usually combining individual sessions and collateral contacts) to help children adjust to placement in foster or group homes.


The STAT program serves children and young adults, ranging in age from just a few days up to their 21st birthday. The program is committed to helping them cope with the traumas of abuse and neglect while working to minimize the impact of being separated from their family, home, and community. In addition, STAT clinicians may carry a small caseload of clients who experience chronic placement instability for whom treatment could extend up to a year.


As a Mental Health Therapist for Foster Youth, you will provide community-based, intensive strategies to address current needs for stabilization, crisis intervention and trauma-focused treatment. We are looking for effective clinicians and collaborators who will provide superb psychosocial assessments, treatment planning, individual and family therapy, and case management.


As a Mental Health Therapist for Foster Youth, you will:



  • Provide screening, assessment and therapeutic interventions as children enter into or change placement within the child welfare system. Consults with county social workers and makes recommendations regarding mental health needs of clients.

  • Provide short-term therapeutic interventions to clients as well as collateral support and guidance to foster parents, relatives and group home staff in order to facilitate children’s transition to and stabilization in their placements.

  • Provide individual and family therapy, caregiver guidance, collateral contacts and case management to children with chronic placement instability for a period of 3-12 months (in both English and Spanish). May work with a case manager as part of a team to best serve youth.

  • Provide services at WCC program sites and in the community (youth’s homes, communities, other agencies, and schools); travel is required up to 90 miles one way.

  • Maintain clinical caseload minimum; meets billable hours & goals as set by the agency.

  • Collaborate with other professionals within WCC and partnering agencies to ensure the highest quality of service to WCC clients.

  • Make collateral contacts on behalf of clients as needed.

  • Comply with WCC policies, procedures, and deadlines related to billing, record keeping, and documentation.

  • Participate in agency and program meetings and professional development trainings as required, including: Agency and program- specific meetings (3 hours/week), Individual Clinical Supervision/consultation (1 hour/week), Clinical Case Conference (2 hours/week).


The ideal candidates have…



  • A Master’s or Doctoral degree in clinical or counseling psychology, marriage and family therapy, or social work. Master’s level candidates must be currently registered with the California Board of Behavioral Sciences.

  • Clean CA DMV record and current car insurance and car; travel is required.

  • 1-2 years of post-graduate experience with children/youth.

  • Prior residential or milieu experience preferred.

  • Current California mental health license preferred. (Based on our funding sources, California Department of Mental Health requires all unlicensed psychologists to apply for a professional licensing waiver or show proof of a valid waiver prior to hire date)

  • Proven experience providing individual psychotherapy, parent support, and family therapy to children, adolescents and their families, particularly where children are or have been in foster care.

  • Demonstrated experience working with diverse communities

  • CPR, First Aid, and Bloodborne Pathogens certification is required.

  • Knowledge of de-escalation techniques is preferred.

  • Demonstrated clinical experience with foster care, DSS, and community mental health.
