Position: Mental Health Case Manager, EMIC Behavioral Health Services Reports to: Program Director, Treatment and Gender Specific Programs

Program Summary: The EMIC Behavioral Health Services offered at Horizons provides culturally affirming, population focused, mental health services for TAY youth, ages 16-24, and/or their families. Services include outreach and engagement to raise awareness about the program and services, screening and assessment, wellness activities/groups, individual and group therapeutic services, and case management. This position will serve as the hub for service enrollment, engagement, and coordination; receiving referrals, conducting screenings, connecting clients and/or their family members to both on and offsite services including therapy, faciliate wellness groups, and providing case management to clients which includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services.

Duties and Responsibilities:

• Conduct outreach activities for the purposes of engaging youth in mental health services, including the development of outreach materials and plans.

• Coordinate and oversee the referral process.

• Conduct client screening/intake to ensure that all individuals are adequately and appropriately served according to their individual needs.

• Complete case management assessment on all clients entering caseload and consistently monitor progress.

• Provide on-going supportive and/or case management functions in accordance with the problems, needs and strategies identified within the case plan in order to help the clients achieve the stated goals and objectives. This includes communicating regularly with schools, probation officers, social worker, family members and other caregivers, regarding client’s treatment progress, etc., when appropriate.

• Document and maintain up to date client files while ensuring confidentiality, according to clinical procedures.

• Act as an advocate for clients and families to ensure service delivery.

• Develop and facilitate wellness groups and activities.

• Accumulate knowledge of, and coordinate services with other providers, when appropriate.

• Connect families with needed and available community resources, follow-up with clients and agencies as appropriate to document use/success of referral.

• Participate in continuing education activities/trainings, remaining knowledgeable in area (s) of expertise.

• Attend weekly interdisciplinary clinical meetings with clinical staff and bi-weekly individual supervision meetings with the Clinical Director.

• Adhere to agency policy, procedures and the professional code of ethics.

• Other duties as assigned by Supervisor.

MINIMUM QUALIFICATIONS:

• BA in Social Work and/or related field and/or a minimum of 2+ years working with at risk youth and their families.

• Knowledge and skills in community based behavioral health care (mental health) and case management experience.

• Experience conducting screenings and keeping client case notes.

• Adept in case plan development and tracking.

• Able to develop and facilitate mental health related wellness groups.

• Must be detail oriented, deadline driven, and able to work independently and take initiative.

• Bilingual (Spanish/English).

• Knowledge of youth service providers in San Francisco preferred.

• Knowledge of clinical treatment, healing arts, intervention techniques, and approaches to youth development, behavior modification, harm reduction, etc.

• Experience working with youth within the juvenile justice system, youth with disabilities, youth with co-occurring disorders, and/or gang-affiliated, etc.

• Able to maintain confidential, accurate, and complete records including documentation of daily encounters; progress notes; weekly and monthly reports, etc.

• Excellent organizational, communication, written, and verbal skills.

• Ability to work as a member of a team and willing to be flexible (that may include working evenings).

• Must be able to pass a Department of Justice background check and clear a TB test before first day of employment.

• This position is under the collective bargaining agreement with SEIU 1021 and in such is subject to enrollment.

• If in recovery, must be clean and sober for a minimum of 2 continuous years.

Compensation and Benefits This is a full-time, permanent position after successful completion of a 6-month probation period. Eligible for benefits (medical, dental, vision plan, accidental life and AD&D insurance) on the 1st day of the month following 1 full month of continuous employment. Other benefits include paid vacation, sick leave, and holidays. The hourly wage ranges from $21.25 to 24.62.

Position Title: Principal Substance Use Counselor

Reports To: Executive Director (interim basis)

Program Summary : The Substance Use Outpatient Treatment Program (OP) provides culturally-rooted services that address the needs of youth in our community through individual, group, and family counseling, case management, collateral, and other healing-focused strategies such as art and music therapy, medicinal drumming, etc. Services are provided onsite, in the community including other CBOs, Juvenile Hall and San Francisco County Jails, at participating schools.

Responsibilities:

· Provide culturally and linguistically competent Substance Use Outpatient Treatment Services to youth and young adults ages 10-26.

· Work closely to with SFUSD and other City Department staff to coordinate referrals and service implementation to enhance a client’s experience and success in the program.

· Develop and implement an outreach plan and corresponding activities for the purpose of engaging youth in programming.

· Work collaboratively with Agency staff to increase knowledge and participation in OP services and initiatives, and support the facilitation of, and increase in, referrals and program support.

· Conduct client intakes, screening, and assessments to determine needs, strengths, supports, etc.

· Develop strength-based Plans of Care for each client and consistently monitor progress.

· Conduct individual, group, and family counseling, including follow-ups.

· Document and maintain up to date client files, progress notes, and plans of care while ensuring confidentiality, according to clinical procedures.

· Track client services and enter them into the AVATAR system on a daily basis, and within 24-48 of service provision.

· Act as an advocate for clients and families to ensure quality and responsive service delivery.

· Provide case management and collateral services that includes communicating regularly with schools, probation officers, social workers, family members and other caregivers, regarding client’s treatment progress, etc., when appropriate.

· Accumulate knowledge of, and coordinate services with other providers, when appropriate.

· Provide coordination support to achieve the mission and objectives of the Outpatient Program.

· Attend weekly Encuentro Clinical meetings, bi-weekly All Staff meetings and other funding source and required meetings.

· Attend all required trainings, including DMC ODS, documentation, other training's to stay abreast of program and service requirements.

· Participate in all relevant trainings (ASAM, DMC ODS, documentation, etc) to ensure knowledge and implementation of funding source requirement. · Perform other duties as requested by the Program Director, Treatment and Gender-Specific Programs.

· A minimum of 2.5 years working with at risk youth and their families.

· Must be a State Certified Counselor (or enrolled in an accredited institution and in the process of obtaining certification), through the CCAPP California Consortium of Addiction Programs and Professionals certification credentials in CADC, CATC, or CCAPP.

· Bilingual (Spanish/English) preferred.

· Knowledge in Motivational Interviewing, Stages of Change, Motivational Enhancement Therapy, Cognitive Behavioral Therapy, MET/CBT 5 treatment models.

· Knowledgeable of clinical treatment, healing arts, intervention techniques, and approaches to youth development. · Knowledge and ability to use Avatar system for all documentation.

· Experience working with youth within the juvenile justice system, youth with disabilities, youth with co-occurring disorders, and/or gang-affiliated, etc.

· Able to maintain confidential, accurate, and complete records including documentation of daily encounters; progress notes; weekly and monthly reports, etc.

· Must be a self-starter and able to organize/coordinate services/workflow in a flexible setting.

· If in recovery, must be clean and sober for at least two continuous years.

· Must be able to pass a background check and clear a TB test before first day of employment.

· Must have a valid driver’s license and the ability to operate the agency van.

Education/ Experience:

Description: Our Therapists are responsible for providing clinical documentation to meet all County, State and Federal documentation requirements, and for meeting with the clinical supervisor on a regular basis to meet all Behavioral Board Sciences supervision requirements. Additionally, this person will reappraise original goals and objectives and adapt them to individual consumer needs while coordinating activities, as well at design appropriate activities, materials, and methods for remediation according to individual consumers. Position requirements:

Summary: The primary goal of the Therapist is to help those struggling with interpersonal/relationship issues. The main role is to develop a humanistic and empathetic relationship with those who are struggling. This process will help promote trust and growth with clients who are faced with personal, family and social crisis. Therapists will demonstrate mastery of scope of practice as evidenced by measurable performance evaluations of direct client services and written documentation skills:

Completion of all client process duties and/or documentation within timelines



  • Interns/Associates/Psychological Assistant are required to attend Board of Behavioral Science required supervision provided weekly, to earn hours toward licensure.


  • Licensed Therapist will participate in monthly consultation/peer group supervision.


  • Therapists are to attend weekly clinical program meetings and ongoing/continuous educational training on documentation to medical necessity. This will include mastery of ASAM criteria based on insurance based medical necessity criteria for all levels of care in continuum of care provided in agency.


  • Daily contact per work days assigned, as well as weekly interdisciplinary treatment team attendance, to collaborate with all members of staff to provide continuity of care.


  • Respond to all social service needs, requests by physicians, legal issues, and family needs.


  • Required documentation in chart according to policy time lines to include: bio-psych-socials, master treatment plans, individual, family, group sessions, weekly status review, transition/discharge plans and other clinically indicated documentation.


  • Must possess basic computer skills including working knowledge of both Mac and PC operating systems and demonstrated proficiency with Microsoft Office Suite, and EHR.


  • Therapist’s skills will be measured by management through regular routine quantitative and qualitative chart audits. Chart audits will reflect clinically appropriate content and timeline benchmarks of completion.


  • Clinical updates to family and/or referral sources (as allowed via client consent).


  • Regular sessions with clients, as assigned, conducting individual, family and group therapy.


  • Crisis Intervention.


  • Observe, evaluate and report on behavior, attitudes general emotional stability clients and their family members to help assist in treatment planning/discharge planning by assessing social needs of individual clients, including referrals to outside providers after transition to outpatient therapy.


  • Record all interventions and interactions with the client, family and community agencies through progress notes in the chart/electronic medical record with master treatment plan guiding services.


  • Therapist will sign and acknowledge job description/revised job description which will be placed in employee file.


  • Therapist will be provided performance evaluations to measure performance areas. Inability to meet performance expectations will result in specific, short term performance enhancement action plan with a timeline to improve. Below average rating in employee performance will result in progressive disciplinary action up to and including termination of employment.



 


Minimum Qualifications:


 



  • Minimum 1500 hours earned toward licensing hours with a registered number with a positive standing from the California Board of Behavioral Sciences.


  • Posses a valid license to practice as a Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Clinical Counselor or Licensed Psychologist issued by the California Board of Behavioral Sciences/Board of Psychology.


  • Master’s Degree or PsyD and two years of supervised clinical experience.


  • Experience in substance abuse and dual diagnosis residential treatment facility preferred.


  • CBT and evidence based experience a plus


  • Experience with trauma and eating disorders preferred.


  • Experience with providing prevention and early intervention behavioral health services.


  • Must have treatment experience with strong professional references attesting to high degree of clinical sophistication.


  • Trained to independently diagnose and treat mental health and substance abuse problems.


  • Experience with evidenced based documentation and/or ASAM documentation.


  • Current certification in First Aid/CPR.

SDC is currently seeking dedicated, invested, resilient, and reliable professionals interested in gaining hours toward licensure. Our program involves providing mental health treatment for individuals, families, teens, and couples who are on their journey toward wellness and healing. Clinicians will learn to diagnose and treat clients with mental and emotional disorders by applying various psycho-therapeutic theories and techniques including but not limited to Cognitive Behavioral Therapy, Depth Psychology and Family Systems. Clinicians will also learn to complete intakes, assessments, and progress notes in accordance to legal and ethical guidelines through an electronic health records system. In addition to providing mental health treatment, associates will have the opportunity to learn business management and grow your practice through identifying your ideal client and marketing your skills. Registered AMFT or equivalent – MUST have BBS number! Supervision guaranteed!


QUALIFICATIONS:



  • Master’s degree, or higher, in mental health and a BBS issued California Associate Number

  • Mandatory 1 year experience providing direct service to clients

  • Mandatory existing caseload of 3-5 that will follow with you

  • Bilingual a plus

  • Must commit to a minimum of 1 year and work weekends

  • Must be willing to maintain a caseload of at least 10 clients and minimum 13 hours/week

DUTIES AND RESPONSIBILITIES

1.Uphold and enforce the Soul Development Center’s standards, policies and procedures, client rights, and professional code of ethics and conduct.
2. Conduct clinical evaluation, bio-psycho-social assessment, client diagnosis and progress notes. Prepare client-centered treatment plans, case management, consultation, education, and refer as needed.


3. Provide therapeutic interventions to individuals, groups and families as needed and facilitate individual psychotherapy sessions and group therapy.
4. Provide services in accordance to Soul Development Center policies and procedures, and established Mental Health Standards as well as within the scope of practice of a therapist.
5. Evaluate the individual and family systems as well as assess client’s functioning within work, family, and routines of daily living and identify areas needing continued support, resources and treatment.
6. Develop, implement, and maintain partnerships with other community providers.
7. Maintain Electronic Health Records to reflect course of treatment and provide required documentation.
8. Provide follow-up and aftercare planning services as needed. Expeditiously discharge inactive clients.
9. Provide emergency services during work hours and after hours if needed.
10. Attend staff meetings, supervisory conferences, training, and other activities, which ensure effective and efficient functioning of clinical operations.
11. Adhere to code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism at all times.
12. Exercise awareness in cultural competency, client satisfaction, quality of services, client needs, and overall health and safety of staff and clients.
13. Independently build case load through self promotion, networking, and minimal supervisor referrals.
