The Clinic Nurse (RN) will plan, coordinate, and oversee our day-to-day nursing services. They will support our primary care providers in patient adherence to treatment goals through coordination of care. In addition to clinical support, the RN will help us achieve our administrative goals. Opportunities will be available for professional growth and development through participation in educational programs, review of current literature, in-service meetings, and workshops.    

● Performs timely client/patient telephone advice and triage assessment with clinical competence and judgment for urgent/drop in care; 

● Documents appropriately in electronic medical record; 

● Uses multiple EMR platforms to gather patient information; 

● Responds to emergencies as they occur in a timely fashion with clear delegation to clinic staff; 

● Performs nursing assessment, implements plan of care, and arranges for follow up care for primary care patients; 

● Delegates MA tasks to enhance patient care and clinic flow, and mentors MA staff on competencies 

● Provides patient education on a variety of topics (e.g. HIV, diabetes, hypertension, asthma, nutrition, and other specific diagnostic procedures); 

● Provides team-based medical management according to defined algorithms for patients with chronic diseases; 

● Provides wound care service under supervision of providers and per clinical protocols on site and as part of our street medicine team. 

● Participates in daily Quality Improvement activities; 

● Performs initial and continued assessment of client health status, immunizations, and TB screening based on established protocols;

● Administers adult immunizations, medications, and phlebotomy consistent with current guidelines and established protocols; 

● Provides culturally and racially competent client care in a manner that is both non-discriminatory and non-judgmental; 

● Communicates effectively with client/patient, clinic staff, the public at large and departmental staff in a non-discriminatory manner; 

● Works with other members of the health care team to provide comprehensive primary care services; 

● Participates in pre-clinic daily huddle, identifies high-needs patients for the clinic session; 

● Supervises and trains non-licensed staff working in the clinic (e.g. Medical Assistants) when assigned by clinic management; 

● Covers clinical job duties of non-licensed staff when required by clinic management; 

● Conducts intake appointments for new patients. Proficiently triages patients based on acuity, complexity, and immediate needs to appropriate Primary Care Provider;  

● Links patient to appropriate in-house and outside medical, behavioral, and social services; 

● Supervises vaccine management program in conjunction with Department of Public Health; 

● Conducts and / or supervises administrative aspects of care coordination (eg. prior authorizations, referrals, post-hospitalization discharge); 

● Conducts and/ or supervises medication and vaccines inventory as well as crash cart inventory. 

● Evaluates and executes routine refills and helps navigating prior authorizations according to established work flows; 

● Coordinates treatment plans/procedures for routine services such as pre-exposure prophylaxis (PrEP), drug detox, and STI treatment; 

● Coordinates follow-up care in accordance with treatment plan outlined by Primary Care Provider and interdisciplinary team; 

● Evaluates patients for medication adherence per Primary Care Provider’s treatment plan. Conducts accurate and thorough medication reconciliation; 

● Serves as integral member of the clinical and interdisciplinary team to provide wrap-around services/care for clients in a team-based care model; 

● Work may be split between our two clinic sites and street medicine. Any travel required to fulfill job duties will be reimbursed; 

● Other job-related duties as assigned.     

Minimum Qualifications:  

● Bachelor's degree in Nursing; at least 3 years of experience that can be demonstrated to be applicable to the duties listed in the job description. OR Associate's degree in Nursing; at least 5 years of experience that can be demonstrated to be applicable to the duties listed in the job description.  

● State of California Licensed Registered Nurse or licensure pending as documented by temporary licensure. 

● Maintains current BLS certification. 

● Effective written and verbal communication skills. 

● Must have a philosophy that is consistent with the Mission, Vision, and Core Values of the organization. 

● Ability to organize, plan, and effectively work on an interdisciplinary team.   

Preferred Qualifications:  

● Experience working with people of diverse backgrounds (gender identity, sexual orientation, race, ethnicity, immigration status). 

● Experience working with clients affected by significant challenges with behavioral and/or mental health. 

● Experience applying harm reduction principles in clinical care. 

● Experience working with highly-traumatized populations, using trauma-informed principles of care. 

● Fluency in English and an additional language (e.g., Spanish, Thai, Laotian, Vietnamese or Chinese).

Duties


LVN/LPN will conduct all activities related to screening patients for COVID19: patient registration in EPIC EMR, insurance verification, point of care test specimen collection/processing, notifying patient of results, educate patient on appropriate next steps based on test results. Additional responsibilities include maintaining infection control and lab processing requirements. Testing location will be based at CVS Health locations both inside and outside the store. Required PPE will be available as appropriate. Role requires candidates to screen in patients in a fast-paced environment. Although the role is assigned to one particular location, travel may be required to nearby locations based on screening needs. Contract extension may be possible on a case-by-case basis depending on location and community testing needs. Training and orientation will be provided prior to start.


Experience

ESSENTIAL FUNCTIONS OF POSITION:* Responsible during the shift for the total nursing care of residents in the assigned unit. * Assumes responsibility for compliance with Federal, State, Local, and company regulations. * Explains procedures and treatments and care plan to resident. * Administers prescribed medications and treatments in accordance with approved nursing techniques. * Transcribes physician’s orders. * Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident’s response to care. * Receives/gives report from/to nurse he/she is relieving/coming on and records sufficient information and implements appropriate follow-up action as necessary * Follows established procedures for charting and reporting all reports of incident/accidents for residents and employees. * Conducts frequent rounds to evaluate effectiveness of care delivery. * Prepares equipment and aids Physician during treatment and examination of resident. * Maintains awareness of comfort and safety needs of resident. * Observes resident, records significant conditions and reactions, and notifies supervisor or Physician of resident’s condition and reaction to drugs, treatments, and significant incidents. * Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident. * Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. * Assists in completion of the care plans (acute and long term) and is involved in the care conference as deemed necessary. * Participates, reports to DON/ Administrator and conducts (as appropriate) investigations related to any alleged abuse. * Ensures that documentation is reflective of the care plan and that the approaches on the care plan are being followed. * Assures that assigned CNA’s are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. * Monitors that CNA’s documentation is completed daily. * Takes report form CNA’s to facilitate accurate documentation in resident medical record. * Follows company policy and procedure in the event a restraint must be utilized. * Coordinates residents’ rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. * Identifies residents at risk for nutritional problems and implements preventative measures. * Records new/changed diet orders. Forwards information to the Dietary Department. * Makes beds, bathes and feeds resident, when necessary. * Assures documentation is accurately completed for admission, discharge and transfer. * Supervises and evaluates implementation of the “Resident’s Rights”. * Participates in department or unit quality improvement activities. * Follows facility procedure requesting supplies and charges for items ordered through Central Supply. * Participates and attends periodic in-services and staff meetings. * Perform additional duties as assigned. EDUCATION / REQUIREMENTS:Current State of L VN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. LVN/LPN will conduct all activities related to screening patients for COVID19: patient registration in EPIC EMR, insurance verification, point of care test specimen collection/processing, notifying patient of results, educate patient on appropriate next steps based on test results. Additional responsibilities include maintaining infection control and lab processing requirements. Testing location will be based at CVS Health locations both inside and outside the store. Required PPE will be available as appropriate. Role requires candidates to screen in patients in a fast-paced environment. Although the role is assigned to one particular location, travel may be required to nearby locations based on screening needs. Contract extension may be possible on a case-by-case basis depending on location and community testing needs. Training and orientation will be provided prior to start.


Experience


Minimum of one year related work experience to include working with an Electronic Medical Record, taking vital signs and performing Point of Care Testing.

Education


The Licensed Practical Nurse is a graduate of a school of Practical or Vocational Nursing, approved by the State Agency and/or accredited by the National League for Nursing (Accrediting Commission (NLNAC)) at the time the program was completed by the applicant. The LPN/LVN must have a full, active and unrestricted license as a Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth of the United States or District of Columbia.
