  • Gather materials and ingredients to prepare for classes in advance

  • Teach nutrition education classes in accordance to SFMFB’s curricula


    • * Please note that classes vary in length and audience (youth, adults, seniors), and language needs. Common topics include MyPlate, food groups, portion sizes, limiting salt and sugar intake, increasing consumption of fruits and vegetables, shopping on a budget, choosing whole grains and healthy fats, label reading, etc.

    • Class curriculum will be provided by the San Francisco- Marin Food Bank



  • Maintain all documentation for the programs and work with the Nutrition Education Senior Program Manager to maintain all records for nutrition education grant requirements

  • On occasion participate in large health fairs or community nutrition education events

Qualifications


  • Bilingual language capacity in Spanish/English and/or Cantonese/English is required

  • Professional experience with teaching and/or nutrition

The Registered Dietitian is a vital part of our comprehensive treatment team. They are responsible for all dietary services and interact closely with our clients. The RD is responsible for consulting in a collaborative fashion with the Discovery treatment team to ensure the best standard of nutrition and dietary services are achieved. Responsibilities include dietary support and supervision, nutrition component design, creation of original class protocols, training new RD’s, and working in collaboration with the Program Director to provide dietary employee reviews and support.

 

Responsibilities


  • Consulting and supporting dietary staff at the assigned facility

  • Meeting with clients in one-on-one meetings

  • Analyzing food service providers to ensure quality products at cost effective prices

  • Designing dietary components in compliance with Department of Health, Joint Commission Accreditation standards. This is in collaboration with the Director of Performance Improvement

  • Training new Registered Dietitians and other dietary staff

  • Supervising Diet Technicians and culinary staff

  • Attending weekly treatment team meetings

  • Team-building exercises and activities

  • May be required to travel to other facilities to attend trainings

Qualifications

  • Eating disorder experience preferred

GENERAL SUMMARY:

Under general supervision and according to The Health Trust’s general policies and procedures, performs moderately complex duties including preparing/creating correspondence and reports; provides customer service to clients, provides assistance with securing food sources, distributes surveys and analyses data in a timely manner, home visits to clients, uses discretion and judgment when screening clients; assists in client outreach and responsible for client management.

PRINCIPAL DUTIES AND RESPONSIBILITIES:


  • Upholds HIPAA office procedures and confidentiality.

  • Customer/Client duties:


    • Provides information to potential and existing clients, staff and volunteers, meal preparation sites and all other contact persons.

    • Provide support for Meals on Wheels food delivery, client management and customer service.

    • Distribute and collect client referrals, surveys on social isolation, general program satisfaction and client hospital stays.

    • Collect and track client data and analyses for semi-annual and annual reports.

    • Works with dietician to determine meal adjustments and diet.

    • Research appropriate food vendors for various food access programs

    • Work directly with clients to determine food security needs.

    • Assist with delivery logistics.

    • Track data and prepare reports as needed.

    • Prepare invoices and track program costs and other related items.

    • Assist Program Manager and Program Director with reports, data collection and other general information as needed.



  • Case Management duties:


    • Provides updated information on clients to case managers.



  • Provides clerical duties including:


    • Answering phone and taking messages.

    • Completes analysis of surveys.

    • General office duties including copying, faxing, filing, word processing, client/meal record upkeep.

    • Update forms and other literature as assigned.

    • Generate routine data from “Serve Tracker”/other data bases for reporting performance levels.



  • Represent The Health Trust through participation in community outreach activities.

  • Proactively problem-solve “Serve Tracker” software issues.

  • Performs other related duties as assigned.

EXPERIENCE AND TRAINING:


  • Bachelors Degree preferred or equivalent experience.

  • Minimum two years of progressively responsible administrative/office environment.

  • Experience in community health field preferred.

  • Bilingual English/Spanish or Vietnamese/English a plus.

  • Proficient in Microsoft Office products/software.

MINIMUM KNOWLEDGE AND ABILITY:
