Our mission is to end family homelessness in the San Francisco Bay Area. Established in 1985, Hamilton Families is San Francisco's leading service provider to homeless families, with carefully designed programs to prevent homelessness, provide shelter and stability, return families to permanent housing and support the well-being of children experiencing homelessness. Program and Position Overview

Hamilton Families Transitional Housing in the North of Panhandle neighborhood houses and supports 20 families at high risk for chronic homelessness for up to 18 months. Families receive intensive social services to identify and address factors contributing to their homelessness to enable them to return to stable, independent housing and economic self-sufficiency. The program provides case management, employment readiness activities, job retention support, intensive housing search support and substance abuse recovery support.

This position will provide direct mental and behavioral health support to children, youth, adults and families who are residing at the Transitional Housing program or participating in the afterschool program on-site (Hamilton Out of School Time- HOST). This position works closely with Family Treatment Court of the Superior Court of San Francisco and Family & Children’s Services to support families who are in recovery and have child welfare involvement.

Primary Duties and Responsibilities

• Provides behavioral health, individual therapy services and recovery support to adults, families, children and youth

• Provide individual trauma recovery and family crisis intervention services to program participants

• Facilitate educational and therapeutic support groups on topics including mental health education, stress-reduction, child development, substance use and parenting support among others

• Participate in all weekly staff meetings, case conferences and trainings as required

• Coordinate services with other staff as part of a multi-disciplinary team

• Act as a liaison between Hamilton Families and SF Family Treatment Court and other partners as necessary

• Provide mental health consultation and training to staff

• Build out mental health internship program to provide direct service to youth participating in HOST

• Provide weekly clinical supervision to interns

• Maintain all necessary records related to participant care and administrative requirements

• Other duties as assigned

Qualifications, Skills and Abilities

• Must be CA Licensed LCSW or LMFT (registered with the Board of Behavioral Sciences or Board of Psychology)

• Must meet the requirements for providing clinical supervision as set forth by the CA Board of Behavioral Sciences

• At least two years of experience providing clinical therapeutic services to children, youth, adults and families

• Master’s degree in Social Work, Psychology and/or Counseling required.

• Familiarity with systems of Child Welfare, Mental Health, Substance Abuse Recovery and Homeless services

• Knowledge and experience working with families and youth who have experienced trauma

• Experience working with adults impacted by substance use disorders and knowledge of recovery models and best practices

• Knowledge and experience of Harm-Reduction and Trauma-Informed Care

• English/Spanish language capacity, highly preferred

• Knowledge of and sensitivity to the needs of families experiencing homelessness; able and willing to work with diverse staff and clients.

• Excellent written and verbal communication skills.

• Proficient in Microsoft Office applications including Word, Excel, and Outlook, etc.

• CPR and First Aid certification required within first six months of hire.

• Essential job functions include using a telephone, working at a standard computer terminal; ability to follow reasonable ergonomic accommodations; sit and stand for long intervals; reach, bend, lift, and carry up to 40 pounds depending on position; and walk up and down stairs several times a day.

• Criminal background check, fingerprint imaging, TB (Tuberculosis) clearance, and documentation required post offer.

Position: Mental Health Case Manager, EMIC Behavioral Health Services Reports to: Program Director, Treatment and Gender Specific Programs

Program Summary: The EMIC Behavioral Health Services offered at Horizons provides culturally affirming, population focused, mental health services for TAY youth, ages 16-24, and/or their families. Services include outreach and engagement to raise awareness about the program and services, screening and assessment, wellness activities/groups, individual and group therapeutic services, and case management. This position will serve as the hub for service enrollment, engagement, and coordination; receiving referrals, conducting screenings, connecting clients and/or their family members to both on and offsite services including therapy, faciliate wellness groups, and providing case management to clients which includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services.

Duties and Responsibilities:

• Conduct outreach activities for the purposes of engaging youth in mental health services, including the development of outreach materials and plans.

• Coordinate and oversee the referral process.

• Conduct client screening/intake to ensure that all individuals are adequately and appropriately served according to their individual needs.

• Complete case management assessment on all clients entering caseload and consistently monitor progress.

• Provide on-going supportive and/or case management functions in accordance with the problems, needs and strategies identified within the case plan in order to help the clients achieve the stated goals and objectives. This includes communicating regularly with schools, probation officers, social worker, family members and other caregivers, regarding client’s treatment progress, etc., when appropriate.

• Document and maintain up to date client files while ensuring confidentiality, according to clinical procedures.

• Act as an advocate for clients and families to ensure service delivery.

• Develop and facilitate wellness groups and activities.

• Accumulate knowledge of, and coordinate services with other providers, when appropriate.

• Connect families with needed and available community resources, follow-up with clients and agencies as appropriate to document use/success of referral.

Compensation and Benefits This is a full-time, permanent position after successful completion of a 6-month probation period. Eligible for benefits (medical, dental, vision plan, accidental life and AD&D insurance) on the 1st day of the month following 1 full month of continuous employment. Other benefits include paid vacation, sick leave, and holidays. The hourly wage ranges from $21.25 to 24.62.

Position Title: Principal Substance Use Counselor

Reports To: Executive Director (interim basis)

Program Summary : The Substance Use Outpatient Treatment Program (OP) provides culturally-rooted services that address the needs of youth in our community through individual, group, and family counseling, case management, collateral, and other healing-focused strategies such as art and music therapy, medicinal drumming, etc. Services are provided onsite, in the community including other CBOs, Juvenile Hall and San Francisco County Jails, at participating schools.

Responsibilities:

· Provide culturally and linguistically competent Substance Use Outpatient Treatment Services to youth and young adults ages 10-26.

· Work closely to with SFUSD and other City Department staff to coordinate referrals and service implementation to enhance a client’s experience and success in the program.

· Develop and implement an outreach plan and corresponding activities for the purpose of engaging youth in programming.

· Work collaboratively with Agency staff to increase knowledge and participation in OP services and initiatives, and support the facilitation of, and increase in, referrals and program support.

· Conduct client intakes, screening, and assessments to determine needs, strengths, supports, etc.

· Develop strength-based Plans of Care for each client and consistently monitor progress.

· Conduct individual, group, and family counseling, including follow-ups.

· Document and maintain up to date client files, progress notes, and plans of care while ensuring confidentiality, according to clinical procedures.

· Track client services and enter them into the AVATAR system on a daily basis, and within 24-48 of service provision.

· Act as an advocate for clients and families to ensure quality and responsive service delivery.

· Provide case management and collateral services that includes communicating regularly with schools, probation officers, social workers, family members and other caregivers, regarding client’s treatment progress, etc., when appropriate.

· Accumulate knowledge of, and coordinate services with other providers, when appropriate.

· Provide coordination support to achieve the mission and objectives of the Outpatient Program.

· Attend weekly Encuentro Clinical meetings, bi-weekly All Staff meetings and other funding source and required meetings.

· Attend all required trainings, including DMC ODS, documentation, other training's to stay abreast of program and service requirements.

· Participate in all relevant trainings (ASAM, DMC ODS, documentation, etc) to ensure knowledge and implementation of funding source requirement. · Perform other duties as requested by the Program Director, Treatment and Gender-Specific Programs.

· A minimum of 2.5 years working with at risk youth and their families.

· Must be a State Certified Counselor (or enrolled in an accredited institution and in the process of obtaining certification), through the CCAPP California Consortium of Addiction Programs and Professionals certification credentials in CADC, CATC, or CCAPP.

· Bilingual (Spanish/English) preferred.

· Knowledge in Motivational Interviewing, Stages of Change, Motivational Enhancement Therapy, Cognitive Behavioral Therapy, MET/CBT 5 treatment models.

· Knowledgeable of clinical treatment, healing arts, intervention techniques, and approaches to youth development. · Knowledge and ability to use Avatar system for all documentation.

  • Order of supplies and work with vendors.

  • Keep studio supplied with “special touches” such as fruit for the water and fresh plants/flowers.

About the Agency:

Rape Trauma Services (RTS) of San Mateo County strives to eliminate all forms of violence, with a special focus on sexual assault and abuse. Our scope of services includes a 24-­hour crisis line, counseling, advocacy, accompaniment, rape and other violence prevention education in schools, and outreach to under-­served populations.

It is our strong belief that violence and abuse must be examined on an individual level (e.g., sexual abuse, rape) as well as a collective level (e.g., genocide, war), and in the context of societal inequities (e.g., sexism, racism, hetero-sexism). We also believe that violent behaviors by individuals as well as societal inequities perpetrated on a collective level are, at their core, about unhealed effects of trauma and ensuing cycles of violence.

RTS Staff, Interns, Trauma Counselors, and Therapists work together to provide a variety of support services to child, adolescent and adult survivors of sexual violence in particular, as well as all forms of direct and indirect trauma from violence and abuse.

● Crisis, Short­-term, and Long-­term Counseling

● Individual and Group Counseling

Trauma Therapist Job Description - Primary tasks for this position include but are not exclusive to the following, under the direction of the Counseling Director:

● Collaborate with staff and the Counseling Director for the timely and clinically accurate provision of trauma based psychotherapy in Spanish, both office based and school­-based work (including children, adolescents, adults, families, and support groups);

● Accompaniment and advocacy services to survivors and their families:

● Work closely with Keller Center staff (forensic hospital/interviews) to ensure follow up to survivors/families;

● Assign and perform timely case management, outreach, and documentation tasks

● Participate in supervision/consultation with Supervisor and Mentoring Staff, as experience indicates;

● Provide on-­call crisis line back-­up support as needed and shared with other Staff;

● Collaborate in planning and implementing outreach in San Mateo County;

Program-­Specific Qualifications: Applicants must demonstrate the following qualities, skills, and experiences:

● Graduate degree in a psychology­-related field; licensed or licensed­-track in your field (MFT, MSW, etc.);

● Oral and written fluency in English and Spanish.

● Must be familiar and experienced with using relationship-­based and trauma ­informed care, and be able to demonstrate knowledge of (and experience in) working with at-­risk children, families and caregivers, especially under­-served populations and gender diverse individuals;

● Must complete 80-hour state-certified Sexual Assault Counselor (SAC) training successfully in order to become a program intern. Training will take place before seeing clients;

 The High School Mental Health Counselor is responsible for supplementing counseling support for students whom the counselors identify as needing more regular contact than the school normally provides. The High School Mental Health Counselor collaborates with the Director of Counseling (DoC) to schedule those students for 30-40 minute sessions approximately every two or three weeks. The position is part time, 5-6 hours, one day per week.    

Accountability:

The High School Mental Health Counselor reports to and is accountable to the Director of Counseling and is appointed, supervised, evaluated and dismissed by the Principal in consultation with the Assistant Principal of Academics and the Director of Counseling.

 Member of: Counseling Department and other committees as assigned by the Principal.   

Responsibilities 

● Uphold the mission of Saint Joseph Notre Dame High School and model a moral code which is consistent with the teachings of the Roman Catholic Church.

 

● Meet regularly with all students as assigned by the DoC. 

● Keep confidential written records files and Powerschool logs on all students seen. 

● Confer with the DoC and the students’ assigned counselor for smooth transitions of care and support. 

● Make all required reports as a mandated reporter working with minors, in collaboration with the DoC. 

● Uphold any and all professional and ethical standards, e. g., American School Counselor Association 

● Collaborate with parents and guardians whenever appropriate.

Under the supervision of the Wellness Programs Manager, the Wellness Programs Coordinator builds and maintains a wellness coaching relationship with the Rafiki Wellness Programs participants and assists them through the process of actively working towards better health by providing support, encouragement education and assisting clients with building agency and building self-confidence. The Wellness Programs Coordinator also leads in the support of the day-to-day administrative, project management of the Wellness events including logistical support activities for the First Saturday monthly event, the Rafiki Wellness Passport Program, some aspects of the annual Health and Healing Summit, and other designated Wellness programs. The Wellness Program Coordinator will also lend some oversight of the agency’s wellness and nutrition contracts and the related reporting tasks. The Wellness Programs Coordinator acts as a primary point of contact for both internal and external constituencies; assists the Wellness Programs Manager as needed, in communicating with constituents; and handles multiple incoming issues and concerns related to Rafiki’s Wellness contracts.

Essential Duties:

The Wellness Programs Coordinator will provide program logistics and administrative support for Rafiki Wellness programs in these areas, including but not limited to:

Rafiki Coalition Wellness Programs

Provide onsite support to Rafiki Wellness Programs and participants in order to help them identify a need for behavior change to improve health status, reduce health risks and improve quality of life.

Follow-up with program participants to ensure that wellness program engagement has been achieved and assess future educational and/or program referral needs. Accurately document interaction and interventions.

Enter data (e.g., activities and demographics) and monitor the programs using the Rafiki Wellness Passport Program dashboards (i.e., metrics databases). Produce monthly reports in order to monitor individual and overall program progress in achieving goals.

Provide professional administrative and logistical support to the Wellness Programs Manager.

Answer calls, provide information, take messages and represent the agency to callers.

Generate correspondence as needed. Perform accurate word processing and budget monitoring.

Develop and maintain meeting minutes, meeting notes and work binders/notebooks that relate to Wellness Programing.

Set up and maintain manual and electronic filing systems.

Complete monthly Unit of Service reporting

Procuring of practitioner invoices and creation of payment vouchers

Maintain relationships and negotiations with vendors and practitioners to the wellness programs

Scheduling of vendors and practitioners for Wellness programs along

Project lead for Rafiki Coalition’s Passport to Wellness Program.

First Saturday Health Party for the Community (Project and Event Management)

· The Wellness Coordinator is expected to oversee all aspects of the First Saturday monthly event and enlist assistance and support when needed from the Wellness Program Assistants and the Wellness Program Manager.

· Provide logistical support, marketing, data tracking

· Provide oversight of administrative tasks and event planning related to monthly health party

· Provide oversight of the nutrition portion of the event including negotiating with Chefs for the event

· Assign staff members and volunteers for roles during the event.

Create and maintain all marketing materials and flyers related to programming.

Partner with and support the Patient Navigation and Wellness Clinic teams.

Provide client coaching and support.

Guide and provide support to nursing staff providing health screenings to clients.

Develop in the skill of providing formal and ad-hoc trainings as required to clients, partners and fellow staff.

Reporting and creation of summary for events and other tasks as needed.

Program Outreach

Coordinate and schedule volunteers for Wellness events, focus groups, mailings and other Wellness activities.

Coordinate mass communications, mailings, and social media postings with input from staff. Tasks involve preparation, emailing and all implementation related to outreach communications.

Support the agency’s participation in community health fairs and potentially meetings with partners and funders.

Report out on meetings on a regular basis with community partners and other related business partners when assigned as the agency’s representative followed by written summaries to Wellness Program Manager.

In addition, the Programs Coordinator will perform additional and miscellaneous job-related duties as directed by the Wellness Program Manager, Director of Programs and/or Executive Director in support of the mission, goals and objectives of the agency.

Preferred Qualifications:

· Bachelor’s degree and or at least 3-5 years of experience that is directly related to the duties and responsibilities aforementioned or outlined in the associated contracts.

· Experience in holistic/wellness practices is a plus.

· Computer literate, including Microsoft software (Word, PowerPoint, etc.) and social media platforms. Big plus for Medical Record Systems experience.

· Sensitivity to the realities of health disparities, specifically in the Black community.

· Experience working with a diverse staff and constituencies.

· Good verbal, written and interpersonal skills.

· Comfortable with public speaking.

· Sense of humor and cultivated emotional intelligence skills.

  • Experience facilitating a range of meetings including collaborative planning and community engagement for highly visible services and initiatives
