        Role Description:


        We’re seeking to fill the role for an RN Health Coach Director. The role will support and engage with patients to help them achieve an optimal level of health and maintain wellness in light of new or existing conditions such as diabetes, hypertension, cardiovascular disease, weight status, and more. The ideal candidate will establish clinical protocols, create and compile educational materials, build and manage a team of Care Coordinators and excel in directly coaching patients to meet Care Plan specifications.]


        This role and subordinate roles will provide thorough education about the patient’s disease process, lifestyle changes, medication regimens, diet and exercise, and work with the patient to overcome roadblocks. All activities are completed with the patient virtually via our software.


        The RN Health Coach Director will manage a team of Care Coordinators (Medical Assistant Health Coaches) and make clinical judgements, assessments, and decisions for their patients such as directing them to a specific point of care, making assessments and clinical judgements based on health data, history, and vitals, creating a plan of action following the Care Plan established by the provider, and making on-the-spot judgements for Care Coordinators.


        We expect RN Health Coaches to work at the top of their license. Experience with clinical informatics and health data is preferred. The ideal candidate will have a strong skillset for analyzing patient data and making appropriate clinical determinations with a compassionate, motivating, and discerning personality to deliver guidance to the patient and educate them along the way.

        • Collaborate and coordinate physician development activities in conjunction with other individuals and departments


        Team



        • Oversees the team of Care Coordinators

        Lifestyle Coaching



        • Educate patients on physical activity and nutrition leading to better lifestyle management

        • Direct clients to resources including PF network of fitness specialists, nutritionists and other referral sources

        • Comfortable suggesting, cross-selling, up-selling and following up with interested patients

        • Analyze a patient’s medical history to determine the best possible exercise and fitness regimen

        • Perform fitness tests with medical equipment and analyze the subsequent patient data


        Reporting & Data



        • Develop reporting and data collection practices

        • Maintain charting and documentation for each patient


        Qualifications:



        • Minimum 5 years nursing experience required.

        • Bachelor's Degree, equivalent experience in a lifestyle health field (Exercise, Nutrition, or others)

        • Health Coaching/Consultation experience strongly preferred.

        • Health Coaching certification strongly preferred, or desire to obtain one upon employment.

        • Must live a fit lifestyle yourself.
